MR Number: 3-97-0026 Clinton 1 Date: 02/12/97 Clinton,Illinois RI PC Dockets: 50-461 BWR/GE-6 Subject: CLINTON WORK STOPPAGE Discussion: Based on continuing personnel errors, the Clinton Plant Manager called a partial work stoppage effective at noon on February 11, 1997. The facility came out of a week and a half long stand down for similar procedure adherence and personnel performance problems on Wednesday, February 5, 1997. Since February 5, the licensee has identified a series of procedure violations and examples of inattention to detail which indicate that the previous work stoppage had not been fully effective. Work required to support the availability of safety related equipment, and some Critical Path work, are excluded from the scope of the work stoppage. Regional Action: Monitor licensee actions. MR Number: 3-97-0027 La Salle 1 2 Date: 02/11/97 Marseilles,Illinois RI VIA TELCON Dockets: 50-373,50-374 BWR/GE-5,BWR/GE-5 Subject: LICENSEE STOP WORK INVOLVING ELECTRICAL CONTROL SWITCHES Discussion: On February 8, 1997, at 5:00 p.m., the licensee stopped all work associated with the replacement of General Electric single block module (SBM) electrical control switches. The licensee had been replacing and testing 1157 SBM switches to address problems with switch degradation and limited qualified life. During the SBM switch replacement project, the licensee identified several problems which led the Unit 1 Plant Manager to conclude that all work associated with the project should be suspended. The problems identified by the licensee included the following: - The licensee's receipt inspection was not identifying defective and inappropriate switches delivered to LaSalle by General Electric. - Maintenance personnel, who were replacing the switches in the plant, were not consistent in their interpretation of work instructions. This inconsistency resulted in different methods of completing the switch replacement procedure. Specifically, a step in the procedure required the mechanics to request a temporary lifting of the out-of-service. Some mechanics translated that instruction to mean request a temporary lift while continuing with the procedure, and other mechanics translated the step to mean request a temporary lift before proceeding with the work. - Mechanics were not performing the post-maintenance testing of the switches as prescribed by the procedures. When the mechanic could not test the procedure exactly as written, a test exception would be documented as part of the procedure, instead of stopping the test and correcting the procedure. The licensee assembled a team, comprised of engineering, maintenance, operations, and work control personnel, to review the SBM switch replacement -2- project. The licensee does not plan to resume the SBM switch replacement activities until a management review has been conducted and the problems are corrected. Regional Action: For information. Contact: M.
