MR Number: 2-97-0063 Turkey Point 3 Date: 08/12/97 Miami,Florida Dockets: 50-250 PWR/W-3-LP Subject: UPDATE ON TURKEY POINT UNIT 3 TRIP (UPDATE TO MR NUMBER 2-97-0059, AND 2-97-0061 Reportable Event Number: 32703 Discussion: The licensee installed an updated Air Pax Series 300 module and tested the A AFW pump saatisfactorily. The licensee returned the A AFW pump turbine to service on August 9, 1997, and exited the 30 day Technical Specification (TS) action statement. Most probable root cause was determined to be a spurious electrical overspeed trip signal caused by the Dresser-Rand tachometer module (Part #890009A03). A similar event occurred on January 6, 1997. The overspeed trip was not repeatable after 20 tests of the A AFW pump. The licensee eliminated possible root causes of governor stem binding and malfunctions, mechanical overspeed trip malfunction, improper overspeed trip setpoints, and water slug induced overspeed. The updated Dresser module was installed in 1995. The other two AFW pumps (B and C) have the original but obsolete Air Pax model (Dresser-Rand part #890061A20). The licensee's inspection of the defective 1995 Dresser tachometer module (part #890009A03) (which was in the A AFW pump) indicates that it was not manufactured by Air Pax or was modified. Apparently, Vogtle Nuclear Station experienced problems with this same part number module in 1993 (reference LER 50-425-007) and made a part 21 notification report. However, the Turkey Point licensee had not received that information. The licensee is currently evaluating the need for a part 21 notification and considering other industry notifications. Dresser indicates that 12 of these suspect modules exist. Two of them were received by Turkey Point and have been removed from use or spare parts. Additional corrective actions include: - increased the normal monthly A AFW pump surveillance testing (weekly for one month, and then bi-weekly for two months); - maintaining the "C" AFW pump (swing pump) aligned to train 1 of the AFW system for both units (Note: allowed by the Updated Final Safety Analysis Report and Technical Specifications); - investigating/evaluating the possible removal of the electronic overspeed trip device (apparently other plants have done this); and - LER submittal, including possible part 21 notification. Unit 3 and 4 remain at full power with the A and C AFW pumps aligned to train 1 and the B AFW pump aligned to train 2. No TS action statements are presently in effect.
