MR Number: 2-97-0070 Mc Guire 1 2 Date: 09/09/97 Cornelius,North Carolina Dockets: 50-369,50-370 PWR/W-4-LP,PWR/W-4-LP Subject: DUAL UNIT REACTOR TRIP - UPDATE Reportable Event Number: 32883 Discussion: This is an update to the September 6, 1997, automatic trip of McGuire Units 1 and 2 that occurred when the alternate supply breaker to non-safety 120 VAC instrument and control power bus KXA opened; thereby stripping control power to several plant components in both units. Specifically, the resultant loss of main feedwater pump speed control in Unit 1 led to the pumps tripping and a subsequent main turbine/reactor trip. Affecting different components in Unit 2, the control power loss in that unit caused main steam isolation valves to close and rendered the primary power operated relief valves inoperable, which resulted in a reactor trip on pressurizer high pressure. (The affected circuits were control power only; nonsafety-related. See PN 2-97-51 for further information.) Under normal conditions, an inverter provides two sources of power to bus KXA through an automatic transfer switch. At the time of this dual unit reactor trip event, bus KXA was being powered from its alternate regulated power supply because its normal inverter supply had been taken out of service in support of associated battery testing and equalizing charge. Post-trip investigation determined that a loose lug was the cause for the tripping of the alternate supply breaker to bus KXA. Accordingly, the licensee replaced this breaker and checked similar lug connections in the plant. Having restarted at 5:05 a.m. on September 9, 1997, Unit 1 will be held at 15 percent power to make feedwater control adjustments before continuing on with power escalation. The licensee is currently planning to restart Unit 2 later today.
