NEW ISSUE. The vendor reports that energy redistribution factors used in 
large-break loss of coolant accident analyses do not directly reflect the
effects of moderator voiding.  Specifically, the factors are             
underestimated by 0.5-1.5 percent, corresponding to a 20-60 F increase in
peak clad temperature.  The following units are affected: Palo Verde 1,  
2, 3; San Onofre 2, 3; Waterford 3; Arkansas 2; Calvert Cliffs 1, 2; and

isolating control of essential equipment from the main control room and  
activating it from the remote shutdown panel in the postulated event of a
fire in the main control room. This issue potentially affects all        
circuits routed through the remote shutdown panel. The licensee          
identified this condition for Division 1 emergency diesel generator feed 
circuit breaker 252-DG1KA and the Division 1 essential switchgear heat   
removal system fan damper.

NEW ISSUE. The Clinton licensee reports a misaligned arc chute, part     
number 567F417G02, in Model 50DHP350 4160-volt circuit breakers. Due to  
the inadequate clearance during installation, excessive force would be   
exerted on the line pole unit to attach the arc chute to its front       
support. The licensee had five potentially affected arc chutes and is not
aware of other facilities that may be affected.
