MR Number: 4-97-0075 South Texas 1 Date: 10/07/97 Wadsworth,Texas Resident Inspectors Dockets: 50-498 PWR/W-4-LP Subject: UNIT 1 STARTUP FOLLOWING REFUELING OUTAGE Discussion: On October 4, 1997, at 2:33 p.m. (CDT), the licensee completed their seventh refueling outage for Unit 1 (1RE07). Major activities associated with the outage included steam generator inspection, control rod drop testing, high-pressure turbine refurbishment, reactor coolant pump motor replacement (one motor), vital battery replacement (two banks), and refueling. The licensee inspected approximately 4800 tubes in each steam generator and plugged a total of 154 tubes. The licensee still meets their current reactor coolant system flow and volume Technical Specifications. The licensee conducted control rod drop tests with the refueled reactor at cold and hot conditions. All control rods met their Technical Specification rod drop times and went to rod bottom. The guide tubes and cladding in the reload fuel contain ZIRLO. This change is intended to remediate the phenomenon of incomplete rod insertion (IRI) experienced with previous STP cores. On October 3, 1997, while in Mode 2, the licensee discovered a 20 drops/min. leak from one incore instrumentation seal table mechanical connection. During further heatup of the RCS, the leakage dropped to about 6 drops/min. The licensee subsequently performed a Furmanite repair which stopped the leakage. On October 6, 1997, a safety relief valve on a moisture separator-reheater unexpectedly opened with the reactor at 74 percent power. The licensee subsequently reduced power to take the turbine off-line and affect repairs. See PN 97-054 for further details. The outage lasted about 21 days, 14 hours.
