Date: 11/07/97 Birmingham,Alabama Subject: TRANSPORTATION ACCIDENT INVOLVING RADIOACTIVE PHARMACEUTICAL Discussion: At 4:00 a.m., CST, on November 6, 1997, a licensee vehicle transporting radioactive pharmaceuticals apparently hydroplane on wet pavement on Interstate-20 in the vicinity of Lincoln, Alabama, and overturned at mile marker 168. The radioactive cargo consisted of four packages, each containing eight lead containers, containing unit doses of 10 to 15 microcurie Technicium/Thallium. Local fire and police departments responded immediately. Subsequently, at 5:00 a.m. CST, a State Health Department Inspector responded to conduct radiological assessments and appropriate surveys. Interstate-20 remained closed for approximately one hour until these radiological assessments were completed and the vehicle and packages were secured. The driver was slightly injured, and although three of the four packages were thrown from the vehicle, all of the packages, as confirmed by the State Inspector, remained intact and there was no release of radioactive materials or spread of contamination. The licensee typically shrink wraps the lead containers in a foam liner, as was the case with this shipment, which enhances the overall security of the materials during transit. After all of the clean up, licensee representatives retrieved the four packages and returned them to their Birmingham, Alabama facility. Regional Action: No follow up action is required. Contact: R. MR Number: 3-97-0111 Byron 1 Date: 11/07/97 Byron,Illinois RI VIA PHONE AND PC Dockets: 50-454 PWR/W-4-LP Subject: UNIT 1 SHUTDOEN FOR STEAM GENERATOR REPLACEMENT AND REFUELING OUTAGE Discussion: On November 7, 1997, at approximately 12:30 a.m. (CST), the Unit 1 main turbine was manually tripped. At 1:15 a.m., the reactor was manually tripped from about 3 percent power beginning outage B1R08. In addition to refueling, turbine inspections, and other outage work, the steam generators will be replaced. Unit 1 is scheduled to return to service in early February 1998. Regional Action: The project engineer observed significant portions of the shutdown. The resident inspector will continue to monitor the outage and Unit 2 operations. Contact: N.D. MR Number: 4-97-0085 South Texas 1 2 Date: 11/07/97 Wadsworth,Texas Licensee telecon w/Regional Staff Dockets: 50-498,50-499 PWR/W-4-LP,PWR/W-4-LP Subject: STEAM GENERATOR MANUFACTURING STOPWORK ORDER Discussion: On November 6, 1997, the licensee discovered that four replacement steam generators at the Westinghouse fabrication facility in Pensacola, Florida had the feedwater and auxiliary feedwater nozzle cutouts misoriented. A foreign vendor misoriented the nozzle cutouts 180 degrees because of a design error on a Westinghouse-supplied fabrication drawing. The foreign vendor who fabricated the steam generator shell sections was Ansaldo Energia, Italy. Foreign vendors have fabricated many steam generator components for assembly by Westinghouse at Pensacola. The licensee has issued a world-wide stop work order on steam generator components because of this and other problems with Westinghouse's manufacturing controls.
