MR Number: 1-97-0058 Ginna 1 Date: 11/06/97 Ontario,New York SRI PC Dockets: 50-244 PWR/W-2-LP Subject: QAIN STEAM PIPING CRACKS Discussion: During ISI inspections conducted as part of the current refueling outage of the B-main steam piping inside the containment, an 18 inch long crack was discovered in the pipe-to-gusset fillet weld. The axial crack runs the entire length of the gusset and extends more than 5/8 inch deep into the pipe wall. Pipe wall thickness is 1-1/4 inch. Two smaller cracks at other gusset weld locations on the same pipe were also found. The ISI inspection scope has been expanded to include the A-main steam piping and both trains of main feedwater piping which have similar gussets. The licensee's plans for repairs and potential reportability have not been finalized pending completion of the additional ISI inspections. The licensee will preserve portions of these defects for laboratory analysis and root cause evaluation. Regional Action: Resident inspectors are monitoring licensee activities. Licensee Subject: SHIPMENT OF IODINE-125 SEEDS MISPLACED Discussion: Tennessee, an Agreement State notified NRC on the afternoon of November 5, 1997 that their office had received notice from the Federal Express facility located in Memphis, Tennessee of a missing lead pig containing 68 millicuries of iodine-125 seeds. The lead pig containing the sources became seperated from the outer shipping package during shipment on November 1, 1997. The event was not discovered until November 5, 1997. A Federal Express supervisor reported to the State today that the lead pig had been found in the Memphis facility, and that the lead container and sources were intact, undamaged, and the sources had been repackaged for return shipment to the originating Medi-physics facility located in New Jersey. MR Number: 3-97-0110 Byron 1 Date: 11/06/97 Byron,Illinois RI VIA PHONE AND PC Dockets: 50-454 PWR/W-4-LP Subject: STEAM GENERATOR REPLACEMETN PROJECT HEAVY LOAD DROP Discussion: On November 5, 1997, at approximately 8:50 a.m. (CST), a 57,000 pound load was dropped outside near the Unit 1 containment at Byron Station. The load was a 46 foot portion of the runway for the steam generator replacement outside handling system. No personnel were injured. Initial indications are that the load was approximately 70 feet in the air and the crane operator was stopping the ascent. The load free fell about 15 feet until the operator was able to engage an emergency brake. The sudden stop apparently caused all four slings to break, first two and then the remaining two. The runway landed on a road and was severely damaged. A portion of the tower was staged nearby and struck when the runway landed. Several pieces of the tower will require replacement or repair. Bechtel is conducting a root cause investigation and no official cause has been identified. The area is quarantined. A crane inspection is planned. Currently the licensee is evaluating repair plans, pending root cause identification. Regional Action: The resident inspector responded to the scene and attended the initial fact finding session.
