The organizational changes will eliminate 148 positions, but will actually affect 79 employees, since many of the positions were not filled. Six of the 79 employees affected by the staffing reduction have the option of taking another position within the organization. Regional Action: For information only. MR Number: 2-97-0088 Mc Guire 2 Date: 12/05/97 Cornelius,North Carolina Dockets: 50-370 PWR/W-4-LP Subject: MCGUIRE UNIT 2 AIRLOCK DOOR SEAL TAMPERING Reportable Event Number: 33349 Discussion: On December 4, 1997, with Unit 2 in Mode 6 at the end of a refueling outage, the licensee discovered indications of potential tampering with the Unit 2 upper and lower personnel air lock seals. The airlock design incorporates two inflatable seals per door, for a total of four seals per airlock. The damage was identified during the performance of required testing of the air lock seal integrity to support restart of the unit. It appeared that a sharp instrument was used to puncture or slash approximately 1 to 2 inch long indications on the hinge side of the door seals. The licensee promptly informed the resident inspectors of the indications and regional management was notified. The residents responded to the concern by immediately inspecting both the upper and lower airlock tampering indications. It was determined that all of the seals had indications (total of eight). Extensive tours were then conducted by the resident staff on December 4 inside the Unit 2 containment. No additional indications of tampering were identified. A telephone call was conducted on December 4 between the licensee, NRR, NMSS, and Region II personnel to discuss the results of the Licensee's investigation and future plans. The licensee formally reported the tampering event to the NRC on December 4. Licensee actions to address the issue included: Heightened awareness of Operations, RADCON, AND Security personnel to this issue; increased housekeeping tours; posting of security guards at the Unit 2 airlocks; initiation of a restart valve lineup; and 100 percent verification of instrument root valve position within the Unit 2 containment. The licensee also completed an initial inspection of the Unit 1 vital areas outside containment. No further indications of tampering were identified. Regional Action: The residents will continue to evaluate the licensee's response to the event and perform additional inspections prior to Unit 2 restart. The residents will also sample Unit 1 standby safety systems and the site's remote standby shutdown facility to verify their integrity with regard to this event. A regional security inspector will also arrive onsite December 5 to further review the licensee's investigation. Contact: ROBERT C. CARROLL (404)562-4511 _ REGION I MORNING REPORT PAGE 3 DECEMBER 5, 1997 Licensee/Facility: Notification: Northeast Utilities MR Number: 1-97-0061 Millstone 3 Date: 12/05/97 Waterford,Connecticut SRI PC Dockets: 50-423 PWR/W-4-LP Subject: CONTAMINATED SILT AND DEBRIS FOUND IN INTAKE STRUCTURE Discussion: The licensee was removing silt and debris from the Millstone Unit 3 circulating water intake structure. They separated the loose debris from the silt, which was then placed in holding ponds and the liquid decanted. The resulting sludge was transported to the local Waterford, Connecticut, landfill. After several truck loads of material were sent to the landfill, the licensee decided to sample the material. The results of two sample counts found 0.1 pico Curies/gm and 0.05 pico Curies/gm of cobalt 60 and cesium 137. [Lower limit of detection (LLD) used for the samples was 0.035 pico Curies/gm. The licensee's ODCM LLD is 0.15 pico Curies/gm]. Approximately 120 cubic yards of material have been placed in the Waterford landfill. Of this material, 60 cubic yards is silt and 60 cubic yards is other material such as mussels and seaweed. The licensee took three samples of the landfill using the 0.035 LLD and found no contamination. They were unable to dump the material back into the bay because of EPA and State of Connecticut silting problems (see 10CFR 50.72 Report No. 33344 dated 12/03/97). The State has been informed of this issue. Regional Action: The NRC is planning to take independent samples of the landfill to confirm the licensee's findings. Any further action will depend on the results of the NRC sample. Region I is coordinating its activities with both the State and NRC Headquarters.
