MR Number: 3-97-0059 La Salle 1 Date: 03/17/97 Marseilles,Illinois SRI VIA TELECON Dockets: 50-373 BWR/GE-5 Subject: OPERATING CREW FAILURE DURING SIMULATOR EXAMINATIONS (UPDATE) Discussion: During the week of March 17, 1997, LaSalle commenced performance evaluations of all operating crews. These evaluations resulted from operator performance deficiencies identified by the licensee during a mid-term training performance evaluation of one operating crew. (See previous Morning Reports 3-97-0046 and 3-97-0054.) Results of performance evaluations conducted to date include: 6 exam crews (corresponding to 3 operating crews) evaluated - 5 crews failed - 1 crew passed 31 licensed SROs and ROs evaluated - 18 failed as individuals - 7 failed due to crew failures - 6 passed Corrective actions for failure, depending upon the individual, consist of one of the following: - short term remediation and re-evaluation - long term remediation and re-evaluation - plans to drop the license Both LaSalle units have been shut down since September 1996. Unit 2 remains in a defueled condition. Regional Action: A regional NRC operator licensing examiner observed the conduct of some of the performance evaluations.
