This bulletin was issued to all medical institution licensees, irradiator licensees, manufacturing and distribution licensees, and calibration service licensees to (1) notify them of the potential for erroneous measurements with certain Victoreen electrometer/dosemeters, (2) request that licensees possessing one of the defective instruments take certain actions, and (3) require that licensees possessing one of the defective instruments provide NRC with a written response. Ramsey, NMSS (301) 415-5746 (301) 415-7887 E-mail: rxa1@nrc.gov E-mail: kmr@nrc.gov _ REGION II MORNING REPORT PAGE 2 MAY 2, 1997 Licensee/Facility: Notification: MR Number: 2-97-0033 Federal Express Date: 05/02/97 Memphis,Tennessee Subject: MISSING RADIOACTIVE MATERIAL Discussion: The State of Tennessee informed the NRC (May 1, 1997) that on April 30, 1997 a one milliliter volume glass vial containing 1.23 millicuries (mCi) of chromium-51 was identified as missing at the Federal Express Facility in Memphis, TN. The FedEx RSO indicated that the event was noted when the vial shield (lead pig) was found during processing, and later the container that housed the shield was located and noted opened, however, the vial was missing. The radioactive material was shipped from Amersham to the University of Texas, M.D. Anderson Cancer Center in Houston, Texas. FedEx continues the search for the vial.
