Those effective July 1, 1997 include: Martin J. McCormick, previously Vice President - Nuclear Engineering, assumes a new position as Vice President - Special Projects, with a primary focus on the New York Nuclear Operating Company (NYNOC). John T. Conway, previously Plant Manager - Unit 2, replaces Mr. McCormick as Vice President - Nuclear Engineering. Kim A. Dahlberg, the General Manager - Projects, replaces Mr. Conway as the acting Plant Manager - Unit 2, until a permanent replacement is determined. Robert G. Smith assumes the position of Operations Manager - Unit 1. Mr. Smith, previously Operations Manager - Unit 2, replaces Michael Balduzzi who resigned. Mr. Donald P. MR Number: 1-97-0040 Maine Yankee 1 Date: 07/01/97 Wiscasset,Maine SRI E-MAIL Dockets: 50-309 PWR/CE Subject: SIGNIFICANT OPERATOR ERRORS WITH VALVE LINEUPS Discussion: On June 28, 1997, Operators inadvertently diverted approximately 10,000 gallons of water from the upender pit (adjacent to the reactor cavity) to the Refueling Water Storage Tank (RWST). This occurred due to an improper valve lineup. Operators were attempting to place the RWST in recirculation in accordance with Operations Procedure, OP 1-11-8, RWST Makeup, Recirculation and Purification using purification pump, P-8. During the system alignment for placing the RWST in recirculation, the operator incorrectly opened the pump's suction valve (CPU-9) from the upender pit instead of the valve (CPU- 24) from the RWST as required by the procedure. This error was not detected until the pump was put in operation and a control room operator noticed the RWST level increasing. The RWST level increased about 10,000 gallons. The evolution was stopped and the valves properly aligned. The licensee has removed the affected operator from shift responsibilities while investigating the event. During another event, on June 26, 1997, a valve lineup error occurred during a planned drain down of the cavity. In that event, operators had failed to properly open a suction valve (CPU-2) to the purification pump as required prior to starting the pump. When started, the pump tripped due to lack of suction flow. The valve was subsequently opened and the evolution completed. On June 30, 1997, a stand down in plant operations was initiated pending results obtained by a review team assembled to investigate the recent events. Meanwhile, only work associated with specified tasks and activities to maintain the plant in safe shutdown were allowed. Regional Action: On July 1, 1997, the DRP Branch Chief held a telephone conference call with Licensee's senior management including the vice-president of Operations, the vice president of Engineering and the managers of Operations, Quality Programs, System Engineering, and the acting Plant Manager.
