Effective April 29, 1997, Mr. Joe Collins, Harris Training Manager, will begin turnover to assume the responsibilities of Maintenance Manager at Harris. Mr. Collins replaces Mr. Bill Gautier as Maintenance Manager. Mr. Gautier will begin reporting to the training organization after the turnover is complete on May 9, 1997. Contact: MILTON SHYMLOCK (404)562-4540 _ REGION II MORNING REPORT PAGE 2 MAY 1, 1997 Licensee/Facility: Notification: MR Number: 2-97-0032 Kentucky Electric Steel Date: 05/01/97 Ashland,Kentucky Subject: KENTUCKY ELECTRIC STEEL Reportable Event Number: 32239 Discussion: On April 30, 1997 a representative from Region II spoke with personnel from the Commonwealth of Kentucky, an Agreement State, Department of Radiation Control, to receive an update on the Kentucky Electric Steel incident reported to the Operations Center on April 29, 1997 (see Event Report No. 32239). Commonwealth personnel indicated that the contaminated material was identified as cesium-137 (Cs-137) and a contractor from Oak Ridge is handling the clean up of the facility. The melt shop has been closed down and the rail car isolated. The Commonwealth's Department of Radiation Control will review and approve clean-up procedures before any clean-up activities can begin. On April 30, 1997, Kentucky sent two of their staff members to the site to perform confirmatory surveys and to collect samples. Upon their return to the office, the samples will be analyzed and an attempt will be made to determine how big the source of the contamination was. MR Number: 4-97-0041 South Texas 2 Date: 05/01/97 Wadsworth,Texas Resident Inspectors Dockets: 50-499 PWR/W-4-LP Subject: PLANT TRIP Reportable Event Number: 32247 Discussion: On April 30, 1997 at approximately 9:58 p.m. (CDT), with Unit 2 at 100 percent power, control room operators manually tripped the unit in anticipation of an automatic trip due to low steam generator water level. All control rods inserted properly following the trip, and all systems functioned as expected. The auxiliary feedwater system automatically started and supplied water to the steam generators. The operators first noted small fluctuations in thermal power which was the result of the Train D feedwater regulating valve oscillating. As the operators went to place the feedwater regulating valve in manual control the oscillations increased causing a decrease of feedwater level in Steam Generator D. The operators were unable to get the feedwater regulating valve to respond to either automatic or manual controls. When the water level in steam Generator D decreased to 35 percent, operators manually tripped the unit in anticipation of an automatic trip at 33 percent level. The licensee's investigation has identified a bad circuit card for the feedwater regulating valve controller. The licensee's investigation is continuing. The unit is currently in Mode 3, shutdown, at normal operating temperature and pressure. Regional Action: The resident inspectors responded to the site to observe the licensee's response to the unit trip and follow up on the licensee's investigation into the feedwater regulating valve failure.
