MR Number: 3-96-0058 Byron 1 2 Date: 05/29/96 Byron,Illinois Dockets: 50-454,50-455 PWR/W-4-LP,PWR/W-4-LP Subject: PRELIMINARY NOTIFICATION OF EVENT (PNO-III-96-031) UPDATE Discussion: The licensee's investigation of the loss of offsite power to Unit 1 on May 23, 1996 determined that the cause of the station auxiliary transformer (SAT) 142-2 failure was moisture intrusion into the "B" phase of the Unit 1 SAT bus duct. Offsite power has been restored to Unit 1 through SAT 142-1. The licensee is investigating water hammer experienced approximately six hours after the reactor trip, during realignment of the main feedwater system, which may have affected 14 pipe hangers and snubbers. The licensee installed a temporary modification to supply electrical power to the OB nonessential service water pump from Unit 2; this pump is normally powered from Unit 1. This arrangement will ensure that each unit has the capability to power a non-essential service water pump. The licensee secured from the Unusual Event at approximately 2:00 p.m., Friday, May 24, 1996. Unit 2 reactor startup is planned for Wednesday morning, May 29, 1996. Regional Action: The NRC Senior Resident Inspector is monitoring the licensee's response to the event. Approximately 13-15 people from several local television and news stations will observe control room activities, the ongoing refueling activites, and discussions about the site's emergency plans.
