Date: 10/29/96 Tampa,Florida License No: 806-1 Subject: PHOSPHORUS-32 SPILL Reportable Event Number: 31232 Discussion: The State of Florida, Office of Radiation Control, notified Region II that a spill was reported at the University of South Florida which is a broad research facility located in Tampa, FL. There were no personnel contaminations as a result of the spill. The spill involved a 65-70 square foot area in a 10 x 10 foot room used only for waste storage at the facility. The contamination was believed to be phosphorus-32 and the radiation level was reported to be 50 millirem per hour at 1 inch above the floor surface. The Licensee's RSO had covered the contaminated area with plexiglass and restricted the area. The spill occurred on Thursday, October 24, 1996 and the State was notified on Monday, October 28, 1996. The State sent a team of inspectors to the site on Monday, October 28, 1996 and reported that the spill was confined to the storage room. The contamination was estimated to be about 100 microcuries of P-32. Contact: R. MR Number: 2-96-0105 Hatch 2 Date: 10/29/96 Baxley,Georgia Dockets: 50-366 BWR/GE-4 Subject: UNUSUAL EVENT DUE TO RELEASE OF TOXIC GAS Reportable Event Number: 31230 Discussion: The licensee reported an Unusual Event at 12:20 p.m. on October 28, 1996, due to the release of a toxic gas. While a vendor was filling the nitrogen tanks in the Nitrogen Storage Tank Room, the nitrogen pressure to the recently-installed relief valve exceeded the valve's setpoint (145 psi), causing the relief valve to open for approximately ten minutes. The nitrogen was secured, the relief valve closed, and the area was purged. There were no personnel injuries or equipment damage due to this event. Procedure 34S0-T48-002-2S, "Containment Atmoshpere Control and Dilution System," was being used during the filling process. The procedure indicated that the vendor use a pressure greater than that of the tank pressure in order to fill the tank. The tank pressure was in its normal range of between 120 to 130 psi; specifically, it was 128 psi. The normal pressure of the nitrogen in the vendor's delivery truck is between 150 to 170 psi. Both the Auxilary Operator and the System Engineer were observing the related work activities at the time. The licensee notified State and Local agencies and the NRC Resident Inspectors. The licensee continues to evaluate the event to determine its root cause. Regional Action: The Senior Resident Inspector monitored the situation until its conclusion at 1:00 pm, upon termination of the release and purging of the area of the nitrogen. The Senior Resident Inspector is following up on the licensee's root cause review. Contact: C. Mr. Pelletier comes to NPPD from the Vermont Yankee Power Station where he was most recently Vice President of Engineering. Mr. Pelletier has been at Vermont Yankee since 1973 and has held the positions of Plant Manager, Assistant Plant Manager, and Maintenance Supervisor. Regional Action: For information only.
