None of the breakers were found in an inoperable condition. NYPA was conducting the inspections based on industry information from Vermont Yankee (reportable events number 31004 and 31192). The safety-related breakers are GE 4kV Magneblast type breakers. The degraded condition consisted of instances where the cotter pins were broken, undersized, and in one case missing. The latching pawls allow the charging motor to charge the closing springs and hold the mechanism in a standby status until the breaker is closed. Failure of the cotter pins could allow the latch pawl pin to dislodge and potentially lead to charging motor failure. This could prevent the breaker from operating as designed. The plant is currently in cold shutdown and in Day 31 of a planned 45 day refueling outage. NYPA is currently replacing the cotter pins in the safety- related breakers which are required to operate in the closed direction. Regional Action: Resident inspectors are continuing their review of this finding and following ongoing corrective actions in this matter; DRS has been consulted in this matter. MR Number: 1-96-0104 Nine Mile Point 1 2 Date: 11/29/96 Lycoming,New York SRI PC Dockets: 50-220,50-410 BWR/GE-2,BWR/GE-5 Subject: UTILITY REORGANIZATION Discussion: On November 27, 1996, the licensee announced the following changes to the Niagara Mohawk Electrical Generation - Nuclear organization: Martin J. McCormick, previously Vice President-Safety Assessment and Support, will replace Mr. Carl Terry as Vice President - Nuclear Engineering. Mr. McCormick had been Vice President-Safety Assessment and Support since January 1, 1994. Carl D. Terry, previously Vice President - Nuclear Engineering, will replace Mr. Martin McCormick as Vice President - Safety Assessment and Support. Mr. Terry was Vice President - Licensing and Engineering from 1986 - 1990, and had been Vice President - Nuclear Engineering since 1990. The changes are effective December 1, 1996. Date: 11/27/96 San Onofre 1 N/A San Clemente,California Dockets: 50-206 PWR/W-3-LP Subject: LOST OF OFFSITE POWER Discussion: On November 26, 1996, at 5:56 p.m., Unit 1, which was permanently shutdown and defueled in 1992, lost all offsite power when the unit's main transformer tripped. Electrical maintenance work had been in progress during the day, but the licensee had not identified a cause for the trip. Power is currently being supplied to the control room and monitoring instruments by unit batteries. The spent fuel pool (SFP) levels and temperature are being monitored manually with operations checks twice per shift. The SFP temperature is 70 degrees Fahrenheit and is increasing at 3 degrees Fahrenheit per day. Without cooling, the SFP temperature would be expected to reach an equilibrium temperature of 105 degrees Fahrenheit in about 2 weeks. Makeup water to the SFP is available by gravity feed, and fire water can be provided from Units 2 and 3. Temporary lighting is being installed in stair wells and compensatory security measures have been implemented because of the loss of security lighting. The loss of power has also caused the loss of control power to 5 of 16 offsite emergency sirens and loss of power to the meteorological towers. The power loss has also taken out of service the pumps to the station sewage treatment plant, and the licensee is reviewing options to provide portable toilet facilities. The licensee is troubleshooting the problem and believes that power will be restored later in the evening. Regional Action: Monitor licensee activities to restore offsite power. Contact: D.
