MR Number: 1-96-0086 Pilgrim 1 Date: 09/27/96 Plymouth,Massachusetts SRI PC Dockets: 50-293 BWR/GE-3 Subject: LOSS OF SHUTDOWN COOLING Reportable Event Number: 31058 Discussion: At 14:18 on September 25, 1996, a loss of shutdown cooling occurred for 40 minutes with a resultant increase in moderator temperature of 12 degrees Fahrenheit when the primary containment isolation system (PCIS) inadvertently actuated providing a Group III isolation. A Group III PCIS actuation isolates (setpoint of 70 psig) the shutdown cooling mode of the residual heat removal system to prevent overpressurization of the low pressure piping. Operators acted swiftly to restore shutdown cooling. The reactor coolant system temperature reached a high of 111 degrees Fahrenheit. The plant had been shut down on September 17, 1996 to facilitate repairs to the "B" reactor building closed cooling water (RBCCW) heat exchanger as described in NRC Preliminary Notification I-96-066, dated September 18, 1996. At the time of the loss of shutdown cooling, instrument and controls (I&C) technicians were in the process of backfilling a feedwater reference leg with a temporary source of pressurized water. The licensee backfills the feedwater reference legs routinely prior to restart to purge any entrapped air in the reference leg water. Based on preliminary review, an inadequate procedure caused the Group III isolation. The water source pressure exceeded the 70 psig setpoint reaching a maximum of 110 psig. The logic for the isolation signal is one out of two taken once. The licensee initiated a procedure change to prevent future recurrence. Regional Action: The senior resident inspector responded to the control room to independently verify restoration of shutdown cooling and notified Region I managers. A detailed review will be performed as part of the routine inspection process. Contact: Richard Laura (508)747-0565 Richard Conte (610)337-5183 _ REGION II MORNING REPORT PAGE 2 SEPTEMBER 27, 1996 Licensee/Facility: Notification: MR Number: 2-96-0089 Puerto Rico Health Department Date: 09/27/96 San Juan,Puerto Rico Subject: Non-Radioactive Item With Radioactive Label Discussion: On September 23, 1996, the Puerto Rico Health Department (PRHD) responded to a schoolteacher's request for assessment of a hard foam jacket displaying a "caution radioactive material" label, used by a student as drink bottle insulator. The school was located in the San Juan area. PRHD surveys showed no radioactivity on the container. PRHD recognized it as a typical overpack for a DuPont Pharma Tc-99m generator and informed NRC, Region II. The only identifying mark was a part number, not traceable to any particular user. Region II discussed the issue with Dupont representatives, who stated that there was insufficient information to identify the licensee who had been shipped the generator. PRHD determined that the student's mother had two more of these overpacks, but she would not reveal the identity of the street seller.
