MR Number: 4-96-0022 South Texas 2 Date: 02/26/96 Wadsworth,Texas Resident Inspector Dockets: 50-499 PWR/W-4-LP Subject: UNIT OUTAGE TO REPAIR MAIN GENERATOR HYDROGEN LEAK Discussion: On February 23, 1996, 5:12 p.m., Unit 2 licensed operators commenced a power reduction to 8 percent reactor power to remove the main turbine-generator from service. On February 20, control room operators noted that hydrogen usage in the main generator had substantially increased. Since that time, engineering and maintenance personnel had been attempting to locate the source of the leakage. During the afternoon of February 23, the leak was identified as issuing from a crack in the water detection drain line in the area of the main generator bushing housing. The 1/2-inch inner diameter line was cracked 180 degrees circumferentially. Licensee engineers determined that the most probable cause of the crack was vibration. This was based on the identification that the first two line supports out from the generator had not been attached to the pipe. Following a generator hydrogen purge, maintenance technicians replaced a portion of the drain line that contained the crack and reinstalled the pipe supports. A dye-penetrant test was performed of the other small bore piping welds that were affected by the missing pipe supports. Additionally, engineers inspected all small bore hydrogen line and sense line connections on Unit 2 and all accessible lines on Unit 1. All lines were secure with properly connected line supports. Following the generator purge and charging with hydrogen, the licensee intends to place the unit back on line. The licensee is continuing to investigate when the pipe supports were disconnected and why they had not been reconnected. No work has been identified that opened the bushing housing since initial startup on either unit. Regional Action: Routine resident inspector followup.
