MR Number: 3-96-0032 Cook 2 Date: 03/26/96 Bridgman,Michigan Dockets: 50-316 PWR/W-4-LP Subject: UNIT 2 SHUTDOWN FOR REFUELING Discussion: On March 23, 1996, the licensee shut down Unit 2 for a scheduled 45 day refueling outage. The shutdown was initiated by a reactor trip from 20 percent power to verify the capability of the control rods to scram. The unit experienced some equipment problems after the reactor trip including failure of the main generator output breaker to open automatically, a source range nuclear instrument indicated low after energization, and the individual rod position indication for two control rods did not agree with actual rod position. The motor-driven AFW pumps received an automatic start signal when the main feedwater pumps tripped as a result of the opening of the reactor trip breakers (P-4). The turbine-driven AFW pump started as a result of low- low steam generator water level after the trip.
