This notice was issued to all teletherapy licensees to alert them to a recently reported failure of an AECL Theratron 780 teletherapy device to place the source in an exposed position during patient treatment, and the potential for similar failures in all older AECL teletherapy devices that were manufactured before 1985. These include the Theratron T765, T80, and T60 model numbers, and the Eldorado E78, E76, E8, and E6 model numbers. Contact: Robert L. Ayres, NMSS (301) 415-5746 E-mail: rxa1@nrc.gov _ REGION IV MORNING REPORT PAGE 2 DECEMBER 26, 1996 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-96-0129 Washington Nuclear 2 Date: 12/24/96 Richland,Washington REQUEST BY LICENSEE Dockets: 50-397 BWR/GE-5 Subject: NOTICE OF ENFORCEMENT DISCRETION FOR WASHINGTON PUBLIC POWER SUPPLY SYSTEM (NOED NUMBER: 96-4-001) Discussion: On December 20, 1996, NRC Region IV orally granted a request from WNP-2 to exercise discretion not to enforce compliance with the actions required by Technical Specifications for repeated testing of emergency diesel generators after a standby service water pump failed monthly surveillance testing. At 8:01 a.m. (PST) on December 20, with WNP-2 operating at 100 percent power, standby service water Pump A tripped on instantaneous overcurrent during surveillance testing and was declared inoperable. The licensee entered appropriate Technical Specification action statements to restore the pump to service within 72 hours, declare Emergency Diesel Generator DG-1 inoperable, and demonstrate the operability of the remaining alternating current sources within four hours and at least every eight hours afterwards. The licensee successfully demonstrated the operability of Emergency Diesel Generators DG-2 and DG-3 within four hours. The licensee's request to not perform the periodic eight hour tests was based on eliminating unnecessary wear on the diesel generators, a previous request to amend the Technical Specifications consistent with the improved standard Technical Specifications, and specified compensatory measures that would be taken. The licensee returned standby service water Pump A to operable status at 1:39 a.m. (PST) on December 21, 1996, and exited the applicable Technical Specification action statements. No damage to the pump, motor or cabling was discovered. The root cause of the overcurrent trip was determined by the licensee to be a reduced setpoint on the overcurrent relay as the result of a recent reanalysis of electrical protective device coordination.
