MR Number: 1-96-0004 Salem 2 Date: 01/24/96 Hancocks Bridge,New Jersey SRI/PC Dockets: 50-311 PWR/W-4-LP Subject: UNUSUAL EVENT DECLARED DUE TO FIRE IN SALEM UNIT 2 AUXILIARY BUILDING Discussion: At 5:56 a.m. on January 24, 1996, the Salem Unit 2 control room operator received a fire protection alarm for Unit 2 auxiliary building 78 foot elevation mechanical penetration area. Technicians in the auxiliary building reported heavy smoke in that area. At 6:00 a.m. the Unit 2 shift supervisor ordered the evacuation of all personnel from Unit 2 auxiliary building. At 6:15 a.m. the Senior Nuclear Shift Supervisor (SNSS) declared an Unusual Event (UE) due to a fire in the plant lasting greater than 10 minutes. Workers cutting up a section of 30 inch diameter service water pipe with a plasma arc torch ignited the coal-tar lining. At 6:28 a.m. fire protection reported to the control room that the fire was out. At 6:46 a.m. the SNSS terminated the UE. The fire did not cause injuries or significant plant damage. PSE&G planned a press release for the fire and Unusual Event declaration. Regional Action: The resident inspectors were present in the control room during the event. Inspectors observed the 78' mechanical penetration area after plant staff extinguished the fire and de-smoked the area. The inspector verified the reflash watch. The Region I Division of Reactor Safety (DRS) dispatched a fire-protection specialist inspector to follow up. DISCUSSION/W LICENSEE Dockets: 50-295,50-304 PWR/W-4-LP,PWR/W-4-LP Subject: POTENTIOL STRIKE AT COMED FACILITIES (BRAIDWOOD, BYRON, LASALLE, DRESDEN, QUAD CITIES Discussion: On January 18, 1996, the licensee informed the Region III office that Union Local 15 of the IBEW would request strike authorization from its members in a mid-February vote. Strike authorization is the local union's first step in requesting a strike sanction from the international union. ComEd and the union have been negotiating a new contract since early 1995. The previous contract expired in April 1995. ComEd employees have been working under the company's final offer to the union since January 1996, when ComEd informed the union that it considered negotiations to be at a standstill and declared its intention to implement its offer of October 1995. Regional Action: Region III is in the process of reviewing the licensee's contingency plans for each ComEd site in accorance with Inspection Procedure 92709, "Strike Contingency Plans." MR Number: 4-96-0016 South Texas 2 Date: 01/25/96 Wadsworth,Texas Resident Inspectors Dockets: 50-499 PWR/W-4-LP Subject: UNIT RESTART AFTER PLANNED OUTAGE Discussion: On January 23, 1996, at approximately 6:19 p.m. (CST) Unit 2 was taken critical after a 13-day outage to repair excessive vibration problems with the main generator. Reactor power was increased and the plant entered Mode 1 at 11:23 p.m. On January 24, the main turbine was rolled but due to high eccentricity readings was tripped at approximately 6:15 a.m. The licensee's investigation revealed that the high eccentricity problems were instrument related. The turbine was rolled again, with no problems noted, and the generator was synchronized to the grid at 4:33 p.m. During the reactor shutdown, the licensee performed control rod assembly drop tests due to concerns raised during a Unit 1 reactor trip on December 18, 1995, in which all of the control rod assemblies did not fully insert. The rod drop testing was completed satisfactorily, with no anomalies identified. Also, the licensee identified three Moisture Separator Reheat intercept valves with broken valve springs. The springs are only to assist in valve closure, and the valves had passed all previous surveillance tests. The licensee replaced the springs and have sent the damaged springs to their testing facility to be analyzed. Regional Action: The Region and Headquarters continue to review the Unit 1 control rod assembly insertion concerns and the residents will be notified by the licensee on the results of the analysis performed on the intercept valve springs.
