MR Number: 2-96-0036 Summer 1 Date: 04/24/96 Jenkinsville,,South Carolina Dockets: 50-395 PWR/W-3-LP Subject: BROKEN GRID STRAP ON IRRADIATED FUEL ASSEMBLY Discussion: On April 23, 1996, at approximately 10:18 am, a manipulator bridge crane operator identified that fuel assembly K-38 had a broken grid strap. Core off-load activities were suspended with the fuel assembly approximately 80 percent withdrawn from the core. A videotape evaluation revealed that a portion of a grid strap had broken and was hanging from the grid strap immediately below it. During movement of K-38 from above the exposed section of the core plate, the broken strap fell from the assembly onto the top of another fuel assembly. The grid strap piece was recovered with remote operated vise grips and placed in a bucket at the bottom of the deep section of the refueling canal. Fuel assembly K-38 was discharged to the Spent Fuel Pool. Visual inspections of K-38 revealed no other damage to the fuel assembly. The fuel assembly that had been adjacent to K-38 in the core was also inspected. No damage to this fuel assembly was observed. On April 24, at 00:17 am, core off-loading was resumed. The licensee plans to conduct an evaluation into the cause of the grid strap failure. Regional Action: Resident Inspector Followup. MR Number: 3-96-0050 Dresden 2 Date: 04/25/96 Morris,Illinois SRI VIA LAN Dockets: 50-237 BWR/GE-3 Subject: UNIT 2 STARTUP FROM REFUELING OUTAGE 14 (D2R14) Discussion: On April 15, the licensee started up Unit 2 (U-2) and began routine startup testing activities. Due to high temperatures on the high pressure coolant injection system discharge piping, a technical specification required shutdown was completed on April 18. (See Event Notification 30309, dated April 18, 1996, for additional information.) Additionally, multiple minor equipment problems (e.g., steam leaks and nuclear instrumentation indication) were repaired. On April 20, U-2 was restarted and startup testing continued. The main turbine generator was synchronized to the grid at 2:41 a.m. (CDT) on April 25, ending D2R14 which began in June 1995. The licensee planned to continue with startup testing activities and to reach full power during the first week of May 1996.
