MR Number: 1-96-0096 Hope Creek 1 Date: 10/25/96 Hancocks Bridge,New Jersey Dockets: 50-354 BWR/GE-4 Subject: INADEQUATE PEAK CLAD TEMPERATURE TRACKING Discussion: On October 23, 1996, the licensee initiated corrective action deliberation on errors discovered during a licensee-conducted QA audit of the fuel vendor (General Electric - Wilmington, NC). A weakness was discovered in the vendor's tracking of errors or changes in the ECCS evaluation models as required by 10 CFR 50.46(a)(3). Prior to 1996, information sent to the licensee since 1990 indicated that there had been no known impact on the calculated peak clad temperature. Earlier in 1996, two impacts had been reported by the vendor to the licensee and when reviewing the handling of this information during the audit, three additional impacts not previously reported to the licensee were discovered, dating back to 1991, 1992, and 1993. In addition, the audit determined that the vendor had not been tracking the cumulative impact errors or changes on the peak clad temperature as expected by the licensee. The cumulative peak clad temperature impact was previously known to be "0"; however, based on the errors identified during the audit, the value is now raised to "100 degrees". Even with the errors accounted for, the calculated peak clad temperature remains well below the ECCS acceptance criteria of 2200 degrees F. The licensee stated that GE had been submitting annual reports to the NRC on behalf of their BWR licensees since 1990. It was thought by GE that these annual reports met the reporting requirements of 10 CFR 50.46 without further reporting by the licensees. While the audit determined that the cumulative change to the peak clad temperature was in error for the Hope Creek licensee, it was not clear at this time whether vendor reporting to the NRC was either wholly or partially in error, or if other licensees were similarly affected. The licensee's initial review of the errors determined that this event was not immediately reportable; however, 10 CFR 50.73 and Part 21 reportability requirements were still being assessed. Regional Action: The resident inspectors will continue to monitor the status of the licensee's reviews of the audit findings and possible reportability of the errors. MR Number: 2-96-0103 Vogtle 2 Date: 10/25/96 Waynesboro,Georgia Dockets: 50-425 PWR/W-4-LP Subject: FOLLOWUP ON LOSS OF 25 PERCENT OF THE CONTROL ROOM ANNUNCIATORS Reportable Event Number: 31210 Discussion: A Notification of Unusual Event was declared on Unit 2 at 0106 on October 25, 1996 due to an electrical technician grounding one of the annunciator alarm sockets while making a repair. Continued licensee investigations disclosed that more annuciators were out of service than the original 25 percent first reported due to additional annunciators failing in an unlighted condition. A blown fuse found in a 125 volt DC power supply pannel was replaced restoring all the failed annunciator circuits. The plant is currently in a power ascension mode, increasing power from its 79 percent level at which the event occurred. Regional Action: The Acting Senior Resident Inspector reported immmediatedly to the site, verifying that the licensee's procedures and corrective actions for this event were properly executed. MR Number: 4-96-0109 Arkansas Nuclear 1 2 Date: 10/25/96 Russelville,Arkansas Resident Inspectors Dockets: 50-313,50-368 PWR/B&W-L-LP,PWR/CE Subject: PLANT STARTUP FROM OUTAGE AND MANAGEMENT CHANGES Discussion: On October 25, 1996, at 6:34 a.m., Arkansas Nuclear One, Unit 1, closed the generator output breakers, ending a refueling outage. As of 10 a.m., the plant was at 12 percent power. The outage had begun on September 15, 1996, after plant trip on September 12. On October 24, 1996, the licensee announced that Charlie Zimmerman was permanently assuming the position of Unit 1 Plant Manager, and Craig Anderson was permanently assuming the position of Unit 2 Plant Manager.
