Replogle (509)377-2627 _ REGION IV MORNING REPORT PAGE 2 JUNE 24, 1996 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-96-0064 Washington Nuclear 2 Date: 06/24/96 Richland,Washington Resident Inspectors Dockets: 50-397 BWR/GE-5 Subject: CONCLUSION OF REFUELING OUTAGE R-11 Discussion: At 4:20 a.m., June 21, 1996, the licensee completed Refueling Outage R-11 when the main turbine generator was connected to the grid. The outage began on April 13, after a period of economic dispatch which began on March 2. Major work that was accomplished during Refueling Outage R-11 included: (1) refueling with installation of 152 new fuel assemblies; (2) installation of an adjustable speed drive control system for the recirculation pumps; (3) installation of 80 jet pump sensing line clamps; (4) rebuilding and replacement of 20 control rod drive mechanisms; and (5) installation of a digital feedwater pump control system. After completion of testing at 100 percent power, the Bonneville Power Administration has requested that WNP-2 shut down for economic dispatch until at least July 28 because of the current availability of hydroelectric power. Replogle (509)377-2627 _ REGION IV MORNING REPORT PAGE 3 JUNE 24, 1996 Licensee/Facility: Notification: Omaha Public Power District MR Number: 4-96-0065 Ft Calhoun 1 Date: 06/24/96 Fort Calhoun,Nebraska From SRI and RI Dockets: 50-285 PWR/CE Subject: STARTUP FROM FORCED OUTAGE Discussion: On June 21, 1996, at 9:14 p.m. (CDT), the Fort Calhoun Station reactor was taken critical following a 14 day unplanned outage to repair Reactor Coolant Pump 3B. On June 7, Reactor Coolant Pump 3B experienced a failure of the anti-rotation device. The device was subsequently replaced and tested satisfactorily. Initial indications were that a check valve in the oil supply line to the anti-rotation device had been installed incorrectly and had contributed to the failure. The licensee verified the proper installation of the check valves on the remaining three pumps. The licensee was also investigating the circumstances surrounding the check valve installation associated with Reactor Coolant Pump 3B. During structural walkdown inspections during the outage, the licensee identified that pressure relief panels in the south end of the emergency feedwater tank room may not function as designed (Event Notification 30635). The licensee implemented the corrective actions stated in the notification regarding removal of structural interferences and implementing procedure changes to open doors to provide additional vent paths when required. These actions ensured that the structures were within the design basis regarding tornado venting. Additional work performed during the outage included reducing the number of outstanding control room deficiencies and operator work-arounds. As of 8 a.m. (CDT) on June 24, the reactor was at approximately 35 percent power. Regional Action: The resident inspectors will continue to monitor licensee activities. Contact: V. MR Number: 4-96-0066 Callaway 1 Date: 06/24/96 Fulton,Missouri Senior Resident via E-Mail Dockets: 50-483 PWR/W-4-LP Subject: LABOR AGREEMENT EXPIRATION Discussion: The present labor agreement between Union Electric Company and Local 148 of the International Union of Operating Engineers expires on July 1, 1996. The International Union of Operating Engineers represents all the physical workers and operating employees at all Union Electric Company Plants, including the Callaway Nuclear Plant. Approximately 280 employees at Callaway are members of the Union. The Company and Union have already extended the old labor agreement indefinitely while negotiations are proceeding. Historically, this is a common practice. The licensee does not anticipate a labor dispute at this time as a result of the continuing negotiations. The licensee has a strike contingency plan outlining measures to maintain satisfactory control over plant activities in the unlikely event of a strike. Regional Action: The resident inspectors have been briefed by the licensee on the status of negotiations and will continue to monitor the situation for significant changes that could result in strike action. The inspectors have also reviewed the licensee's strike contingency plan and identified no items of concern. Contact: D. MR Number: 4-96-0067 Arkansas Nuclear 1 Date: 06/24/96 Russelville,Arkansas From Resident Inspector Dockets: 50-313 PWR/B&W-L-LP Subject: MAIN FEEDWATER PUMP TRIP Discussion: On June 22, 1996 at 4:05 a.m., Unit 1 Main Feedwater Pump A tripped on overspeed. The remaining feedwater pump supplied water to the steam generators and reactor power was automatically reduced to approximately 40 percent power as designed. A recently installed modification to the integrated control system to handle this event worked correctly. The licensee has preliminarily determined that a memory location in the integrated control system was overwritten due to either a fault in the circuit or the associated computer software. This caused the main feedwater pump overspeed trip. As of 8 a.m. on June 24, 1996, the plant was at 40 percent power. The licensee planned to keep the plant at 40 percent power until the cause of the overspeed trip is understood and corrected.
