MR Number: 3-96-0011 Braidwood 2 Date: 01/20/96 Braceville,Illinois SRI Dockets: 50-457 PWR/W-4-LP Subject: LOSS OF OFFSITE POWER UPDATE (SEE PN # PN-III-96-004 AND EVENT #29852) Discussion: An unusual event was terminated at 3:22 a.m. (CST) on January 20, 1996. The event was due to the loss of Unit 2 offsite power on January 18, 1996. The cause of the event hasn't been determined, but lightning has been eliminated as a cause. A seven day repair time is anticipated for the damaged breaker that supplies one source of offsite power to the two system auxiliary transformers (SATs). The licensee was able to recover one of the two Unit 2 SATs. No time estimate has been established for the repair of the damaged SAT. The recovered SAT is carrying both 4.16 kilovolt engineered safety feature (ESF) buses and two of the four 6.9 kilovolt buses. The other two 6.9 kilovolt buses are normally fed from the Unit 2 unit auxiliary transformers. The single SAT has the capacity to carry all the Unit 2 buses and at least one ESF bus from the other unit. Unit 2 remained at 100 percent power. Both units have exited the 72 hour limiting condition for operation. Regional Action: The resident inspectors are continuing to monitor the licensee's activities. Contact: L.F. MILLER, JR. MR Number: 4-96-0013 Comanche Peak 1 Date: 01/23/96 Glen Rose,Texas Senior Resident Inspector Dockets: 50-445 PWR/W-4-LP Subject: MANUAL REACTOR TRIP OF COMANCHE PEAK UNIT 1 Discussion: This morning report supplements event notification number 29874 and preliminary notification PNO-IV-96-003A which reported a Unit 1 manual reactor trip following a loss of feedwater flow on January 22. The loss of feedwater flow occurred when safety-related Elgar Inverter 1V1EC1 failed during troubleshooting and de-energized Bus 1EC1 which resulted in all feedwater isolation valves closing. The inverter failure occurred when plant personnel replaced a blown fuse in the DC-DC converter power supply card while the inverter was still supplying bypass power through the automatic static switch. The plant computer worked properly during the trip. Following a review of computer information, the licensee noted that Main Feedwater Isolation Valve (FWIV) 2 stroked closed in approximately 32 seconds which was longer than the required stroke time of 5 seconds. The licensee verified through testing that FWIV 2 was stroking closed slower than required and is continuing to investigate. Licensee troubleshooting of Inverter 1V1EC1 has yet to determine the cause of the inverter failure. The licensee is making preparations for a plant cooldown required by Technical Specifications related to the inverter failure. Troubleshooting of Nuclear Instrument N-42 revealed that a circuit card switch had failed. The licensee replaced the failed switch and is investigating the root cause of the failure. Regional Action: The resident inspectors reported to the control room following the trip. Resident and regional inspectors are performing a special inspection to review both the inverter failure related trips on January 17 and 22, and the licensee's corrective actions. Contact: A. T.
