The DOE, NRC (Region II), and state inspectors verified the safety and compliance of each cask at the port at Charleston Naval Weapons Station prior to departure on CSX rail cars. The shipment arrived at the Savannah River Site at 6:15 pm on September 22, 1996. DOE issued a press release on September 23, 1996, announcing the completion of the shipment. Region I has been notified because Edlow International is a Region I licensee. Regional Action: Region II inspectors surveilled the shipment and verified that the shipment met applicable requirements and the participation of local and state law enforcement agencies enroute. MR Number: 2-96-0086 Oconee 2 Date: 09/23/96 Seneca,South Carolina Dockets: 50-270 PWR/B&W-L-LP Subject: OCONEE UNIT 2 HIGH PRESSURE INJECTION SYSTEM ISSUES Reportable Event Number: 31041 Discussion: Two equipment problems occurred involving the Oconee Unit 2 high pressure injection (HPI) system since September 18, 1996 (see EN 31038 and 31041). On September 18, the 2B HPI pump motor failed due to an electrical fault. The cause of the fault has not yet been conclusively determined. The motor windings may have failed or the problems may have been a loose electrical connection on a motor lug. Actions to replace the motor were initiated. On September 21, after delays in coupling the motor to the pump, the licensee placed Unit 2 in hot shutdown as required by TS 3.3.1. Two independent injection flowpaths were not available due to system alignment. The electrical ground fault caused a power surge back through the electrical system resulting in an arc that ruptured a small copper line in the unit auxiliary transformer (T2) gas detection/oil system. Electrical loads were shifted to the startup transformer (CT2) and the transformer oil leak was repaired. At about 5:30 p.m. on September 21, a small leak (characterized as a low volume spray or mist) was identified in the reactor building on the 2A1 high pressure injection line. The leak was associated with a socket weld on a one inch drain/test line which had been installed during the most recent refueling outage. Initial information indicates that the problem may have been caused by high vibration on the line. Oconee Units 1 and 3 do not have the one inch lines installed. The licensee initially reported the leak as unisolable due to the proximity of the isolation valve to the leak location. A cooldown was initiated and the leak was subsequently isolated. Currently, Unit 2 is at 340 degrees F and 800 psig. The leaking weld was removed and will be examined. The licensee is implementing a modification which removes the one inch piping on all four injection lines. The residents have been reviewing the licensee's actions, including the HPI motor replacement and testing, transformer oil leak repairs, and the injection line repair activities. Contact: L. MR Number: 4-96-0103 Arkansas Nuclear 1 Date: 09/23/96 Russelville,Arkansas Resident Inspector Dockets: 50-313 PWR/B&W-L-LP Subject: HYDROGEN BURN IN QUENCH TANK PIPING Discussion: On September 21 at 3:45 p.m. (CDT), with the plant in cold shutdown during an outage, a small hydrogen burn occurred while welding on a common electromatic relief valve and pressurizer relief valve tail pipe line to the quench tank. The hydrogen burn, apparently caused by the heat generated during the welding, shook the 10-inch tail pipe. The licensee believed that the hydrogen was present in the tail pipe because the electromatic relief valve had been opened to depressurize the reactor coolant system during plant cooldown. The quench tank rupture disc did not rupture as a result of the burn. A walkdown of the associated piping by the licensee and the NRC inspectors did not identify any structural damage. Based on the magnitude of a quench tank level spike, the licensee estimated peak pressure at 50 psig. No radioactive release occurred as a result of this event. The licensee stopped all welding activities on the reactor coolant system until the cause of the event has been resolved. Regional Action: Resident inspectors responded to the site on September 21 and performed a walkdown of the affected areas.
