The NRC is issuing this information notice to alert addressees to the potential for inoperability of tornado dampers because of either inadequacies in damper testing or deficiencies in damper design. LeFave, NRR (301) 415-3285 Thomas Koshy, NRR (301) 415-1176 _ REGION IV MORNING REPORT PAGE 2 JANUARY 22, 1996 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-96-0012 Cooper 1 Date: 01/22/96 Brownville,Nebraska Resident Inspector Dockets: 50-298 BWR/GE-4 Subject: POTENTIAL TO PREVENT EMERGENCY DIESEL GENERATORS FROM FULFILLING THEIR SAFETY FUNCTIONS Reportable Event Number: 29855 Discussion: On January 18, 1996, the licensee made a 4-hour event notification in which they described an existing condition which could have resulted in the muffler bypass valves from opening and possibly preventing the diesel generators from performing their safety functions. On January 15, 1996, during surveillance testing of Diesel Generator (DG) 2, the muffler bypass valve failed to operate. During troubleshooting on January 17, 1996, the licensee found that 1/4-inch diameter tubing to the solenoid-operated pilot valve exhaust port restricted air flow, preventing the muffler bypass valve from opening. Further investigation revealed that this same 1/4-inch diameter tubing had been installed on DG 1. As immediate corrective action, the licensee has removed the 1/4-inch diameter tubing from both DGs. Preliminary investigation into this event found that the 1/4-inch diameter tubing was added during the last refueling outage as part of installation of a major design modification. However the tubing, which apparently been added to address a foreign material exclusion concern, was installed without proper drawings or engineering evaluation.
