MR Number: 1-96-0037 Calvert Cliffs 1 2 Date: 04/19/96 Lusby,Maryland REGION I PC Dockets: 50-317,50-318 PWR/CE,PWR/CE Subject: MOTOR OPERATED VALVE KEY FAILURE Discussion: During routine surveillance testing while Unit 1 was in a refueling outage, a feedwater header isolation motor operated valve (MOV) failed to re-open following a timed closure test. Investigation revealed that the motor pinion gear key had been displaced. The key had been staked to hold it in place; however, the staking was inadequate, in part due to the hardness of the key and shaft to which the gear was attached. About 30 other safety-related MOVs (15 per unit) were identified as potentially susceptible to the same failure. The valves were fitted with operators manufactured by Limitorque Corp with sizes ranging from SMB-0 and larger. BGE examined the three similar feedwater isolation MOVs (there are two per unit) and found no discrepancies. BGE plans to radiograph the remaining safety-related MOVs, in an order dependent on their degree of susceptibility and contribution to plant risk, to verify the keys are in place. Some non safety-related MOVs being refurbished in Unit 1 were also examined, and no problems were noted. BGE's followup investigation determined that the key failure was most likely due to a combination of factors: (1) the material hardness of both the key and the shaft, making staking more difficult; (2) a high vibration environment; (3) high speed of operation; and (4) failure to bevel the key, as recommended by the vendor, to allow greater metal contact. RIII Dockets: 05000151 Subject: RELEASE OF INSOLUBLE RADIOACTIVE MATERIAL TO SANITARY SEWER Discussion: On April 19, 1996, the licensee notified the NRC Operations Center and Region III of a release of water to the sanitary sewer system containing insoluble radioactive material. The notification was made in accordance with Technical Specification 6.8. The release occurred when a portion, approximately 250 gallons, of the contents of a retention tank was pumped to the sanitary sewer. A pre-release sample of the contents using a 0.45-micron filter paper was counted and indicated no detectable activity. The licensee released the contents of the tank based on a determination that insoluble radioactive material was not present and that limits for soluble radionuclides were met. However, post-release samples taken from approximately one-third of the water still left in the tank indicated that insoluble beta-gamma activity on the order of 10-8 microcuries per milliliter was present. Based upon a 250 gallon release, the licensee calculated that 0.084 microcuries (3108 becquerels) of insoluble radioactive material was released to the sanitary sewer. 10 CFR 20.2003(a)(1) requires that radioactive material released to the sanitary sewer system be readily soluble. Regional Action: NRR and the State of Illinois have been notified. A Region III inspector will follow-up during the next inspection. MR Number: 3-96-0048 Zion 1 2 Date: 04/20/96 Zion,Illinois SRI VIA PC Dockets: 50-295,50-304 PWR/W-4-LP,PWR/W-4-LP Subject: SEVERE WEATHER CONDITIONS AT THE ZION PLANT Discussion: On April 19, 1996, at approximately 10:30 p.m. (CDT), the licensee placed the Unit 1 startup on hold after the station experienced heavy rain and winds up to 96 miles per hour which caused a fault in the switchyard and some minor physical plant damage. An electrical fault occurred on an offsite power supply line which caused breakers to open, isolating Unit 1 from Unit 2 on the ring bus. Plant damage included roofing on the following buildings: fuel, turbine, interim radwaste storage facility, new and old service, and the state monitoring facility. In addition, five security cameras were also damaged. The licensee entered the appropriate abnormal operating procedure and determined that an emergency classification declaration was not warranted based on weather conditions. Compensatory measures were taken to ensure that minimum surveillance requirements were met, and following the electrical line fault investigation the breakers were closed. The startup was resumed and at 5:45 a.m. on April 20, 1996, criticality was achieved. Regional Action: The resident inspectors are monitoring repair activities. Contact: L. F. MILLER, JR. Otto Maynard, the Vice President Plant Operations will become the Chief Operating Officer. The Plant Manager (Britt McKinney) will report to the Chief Operating Officer, along with the Integrated Planning and Scheduling Manager, the Performance Improvement and Assessment Manager (Brad Norton), and the Director of Site Support. The System Engineering group will move from Engineering and report to the Plant Manager along with Operations (Chris Younie), Maintenance, Chemistry, and Health Physics.
