MR Number: 3-96-0026 Zion 1 Date: 02/20/96 Zion,Illinois RI VIA PC Dockets: 50-295 PWR/W-4-LP Subject: UNIT 1 SHUTDOWN GREATER THAN 72 HOURS Discussion: On February 18 the licensee initiated a Unit 1 shutdown to inspect the 1B steam generator for loose parts in the reactor coolant system hot leg. In addition, the licensee will inspect the 1A diesel generator to identify any slipped cam lobes; the licensee will install a camshaft modification to prevent any slipped cam lobes in the future. The planned maintenance outage has an expected duration of twelve days. MR Number: 4-96-0021 River Bend 1 Date: 01/31/96 St Francisville,Louisiana Resident Inspectors Dockets: 50-458 BWR/GE-6 Subject: SUPPRESSION POOL OPERABILITY CONCERN Discussion: On January 30, 1996, during Refueling Outage 6, personnel at the River Bend Station identified numerous items in the suppression pool when they initiated efforts to clean the suppression pool. The items found in the suppression pool included small items such as: tape, nuts, washers, bolts, thermoluminescent dosimeters, pens, welding rods; medium-sized items such as: sheets of paper, pieces of cloth, rolls of tape, and scaffold knuckles; and large items such as: plastic bags, a 12-inch by 16-inch by 1/4-inch thick rubber mat, a scaffold board, an 8-foot piece of scaffold, and a 1-foot by 4-foot piece of plywood. The licensee had listed on their inventory sheet approximately 632 separate line items, with some lines listing multiple items, for a total of about 760 individual items identified. Prior to the outage the licensee had only identified 37 items being in the pool since 1992, as required by their inventory control process. The licensee is presently performing an operability evaluation of the as-found suppression pool condition. The licensee had performed a suppression pool cleanup following Refueling Outage 5. Regional Action: An NRC fuel integrity and reactivity inspection had initially followed the licensee's actions. The resident inspectors will review licensee resolution of this issue. Contact: P. H.
