The changes were made to continue the site's transition to two unit operation following the restart of Unit 3. Key changes included the following: Chris Crane, Assistant Plant Manager, will assume direct responsibility for the Operations and Technical Support Departments. Robert Moll, Operations Manager, will assume responsibility for Operator Training. R. G. Jones, Operations Superintendent, will become Operations Manager. MR Number: 3-96-0010 Palisades 1 Date: 01/19/96 Covert,Michigan RI PC/1/19/96 Dockets: 50-255 PWR/CE Subject: SHUTDOWN GREATER THAN 72 HOURS Reportable Event Number: 29844 Discussion: On January 16, 1996, the 2400 volt AC power automatically transferred from the normal power source to the backup power source. Upon investigation of this occurrence, an electrical fault was discovered in the power supply cable to the 2400 volt "D" Bus, which runs in conduit under the turbine building between the Safeguards Bus and Buses "C" and "D." The "C" and "D" Buses are considered the plant's vital power source. The plant was shutdown and taken off-line on January 17, 1996, at 4:50 a.m (EST). Due to the plant shutdown required by Technical Specification, the licensee declared an Unusual Event under its emergency plan. The Unusual Event was terminated at 4:00 p.m. (EST) on January 17, 1996. The licensee plans to make repairs to the power cable and expects to stay shut down about 7 days. MR Number: 4-96-0008 Waterford 3 Date: 01/19/96 Killona,Louisiana Resident Inspector Dockets: 50-382 PWR/CE Subject: VIRUS FOUND ON CONTROL ROOM COMPUTER Discussion: On January 17, 1996, during the performance of a routine monthly check of computers in the control room, the licensee identified a virus on one of the 21 computer terminals. The licensee performed an assessment of the extent of the spread of the virus and confirmed that the virus affected only one computer terminal. This assessment specifically verified that the virus did not affect the core protection calculator (CPC) or the core operating limits supervisory system (COLSS) computers. The licensee also noted that the virus did not affect any data displayed by the affected computer. Based on a review by the licensee, it was determined that the virus was introduced on the terminal when a Windows program was loaded into the terminal by a computer technician. The licensee established that the virus was present on the disk when it was received by the vendor. To ensure that this problem does not recur, the licensee is establishing a program to: (1) check all software for a virus prior to first use of any program and (2) block the computer disk drives to prevent use of the drives by anyone other than authorized personnel. The transition would be completed by February 2, 1996. Bill Counsil had been the Managing Director since April 1993. Vic Parrish, as the Managing Director, will retain his responsibilities as the Vice President-Nuclear Operations. MR Number: 4-96-0010 South Texas 2 Date: 01/19/96 Wadsworth,Texas SRI/HQ Operations Officer Dockets: 50-499 PWR/W-4-LP Subject: DECLARATION OF NOTIFICATION OF UNUSUAL EVENT DUE TO EXCEEDING TECHNICAL SPECIFICATION 3.03 Discussion: On January 19, 1996, at 9:35 a.m. (CST) the licensee declared a notification of unusual event for Unit 2 due to operation outside of the Technical Specification safety envelope. The Headquarters Operations Officer was notified at 10:04 a.m. (CST). The licensee discovered at approximately 8:35 a.m. (CST) on January 19, 1996, that two feedwater isolation valves had been placed in inoperable status at 5:50 p.m. (CST) on January 18, 1996, to perform planned maintenance. This exceeded Technical Specification 3.7.1.7, which currently allows only one valve to be out of service, with the unit operating in Mode 3. Failure to comply with Technical Specification 3.7.1.7 was not detected during shift turnovers at 7 p.m. (CST) on January 18, 1996, and 7 a.m. (CST) on January 19, 1996. This condition was discovered by the licensee at about 8:35 a.m. (CST) on January 19, 1996, and resulted in entry into Technical Specification 3.03, which required action within 1 hour to cool down within 6 hours. Upon recognition of the applicable Technical Specification, the licensee immediately took action to restore one feedwater isolation valve. NRC identified problems with the initial operability test, which the licensee subsequently repeated satisfactorily. Feedwater Isolation Valve A was declared operable at 10:28 a.m. (CST) and the unusual event was terminated at 10:31 a.m. (CST) on January 19, 1996. The feedwater isolation valves out of service were maintained closed during the maintenance activity, except during operability testing, and the planned maintenance only involved adjustment or addition of valve packing rings.
