During the shutdown, control rod drop testing will be performed in response to NRC Bulletin 96-01. Major outage activities include: complete core offload; reactor vessel 10-year inservice inspection, including the lower internals assembly; main unit transformer replacement; 100 percent steam generator tube inspections; and steam generator tube sleeving. The licensee plans on using the plus-point probe for the steam generator inspections. The outage duration is expected to be 55 days. Regional Action: The resident inspectors are on site monitoring licensee activities and specialist inspections by region based inspectors are planned. Smith has been selected as the new Unit 2 Operations Manager. Mr. Smith has been the Unit 2 Work Control/Outage Manager since February 1994. Other NMPC positions held by Mr. Smith include Nuclear Training Manager, Deputy Station Superintendent, and Operations Superintendent. Mr. Smith is a graduate of the US Naval Academy. Jerry B. Helker has been named to replace Mr. Smith as the Unit 2 Work Control/Outage Manager. Mr. Helker has been the Unit 2 General Supervisor Instrumentation and Control Maintenance. Both positions report directly to John Conway, Plant Manager, and are effective immediately. Regional Action: None Contact: Barry Norris (315)342-4041 Rich Conte (610)337-5183 _ REGION II MORNING REPORT PAGE 5 MARCH 19, 1996 Licensee/Facility: Notification: Tennessee Valley Authority MR Number: 2-96-0021 Watts Bar 1 Date: 03/19/96 Spring City,Tennessee Dockets: 50-390 PWR/W-4-LP Subject: MAIN GENERATOR - LOSS OF ELECTRICAL LOAD Discussion: On March 18, 1996, Watts Bar Unit 1 was operating at 42 percent reactor power, 456 megawatts electric with the power ascension test program in progress, when the main generator experienced a loss of electrical load. Initial indications were that the main turbine governor valves closed, causing the loss of electrical load. TVA with Westinghouse assistance has initiated troubleshooting activities, focusing on the main generator voltage regulator and the main turbine electro-hydraulic control system. The initial evaluation of data provides the following sequence of events: -1st stage turbine pressure drops from 288 psi to 80 psi. Steam dumps open. -Operators manually close steam dumps suspecting malfunction. -Steam generator PORVs open, operators reopen the steam dump valves. -Pressurizer PORV opens then recloses. Pressurizer pressure peaks at 2314 psig. -Operators manually trip turbine -Generator reverse power alarm -Field breaker/generator output breakers open -Plant stable, reactor power reduced to 2 percent. Regional Action: A Region II ORAT Team is on site and will be observing licensee troubleshooting activities and independently verifying plant and operator responses to the event. MR Number: 4-96-0027 Diablo Canyon 1 Date: 03/18/96 Avila Beach,California From Resident Inspectors Dockets: 50-275 PWR/W-4-LP Subject: PLANT STARTUP FOLLOWING REPLACEMENT OF AUXILIARY TRANSFORMER Discussion: On March 17, 1996, the licensee completed a short outage for the replacement and testing of the Unit 1 Auxiliary Transformer 1-1. The transformer had been damaged during a fire on October 21, 1995. Unit 1 auxiliary power was out of service for approximately 70 hours during the work. Following restoration of the transformer the licensee performed a reactor startup, achieving criticality at 6:02 p.m. PST. The main generator was paralleled to the grid on March 18 at 4:36 a.m. PST. Reactor power was maintained at 30 percent for a 24-hour "boron soak" of steam generators and the licensee is presently increasing power to 100 percent. In addition to the transformer replacement the licensee accomplished control rod drop time testing in response to NRC Bulletin 96-01 during the outage.
