The current site vice president, Richard Tuetkin, will be replaced by John H. Mueller. Mueller is the current chief nuclear officer for Nebraska Public Power District's Cooper Station. (708)829-9629 _ REGION III MORNING REPORT PAGE 2 JULY 19, 1996 Licensee/Facility: Notification: Department Of The Army MR Number: 3-96-0080 Anniston, Al Army Base Date: 07/18/96 Rock Island,Illinois LICENSEE VIA TELECON 7-18-96 Dockets: 03020107 License No: 12-00722 Subject: POTENTIAL H-3 UPTAKE Discussion: On July 18, 1996, the NRC was notified by the Radiation Safety Officer at Rock Island, Illinois, that a contamination event had occurred On July 9, 1996, at the Anniston Army Depot in Anniston, Alabama. An individual was opening 30 packages containing one 10 Ci muzzle reference sensor per package. He installed these sensors in front of the fume hood rather than inside it, and completed a swipe of each sensor after it was installed. However, he did not count the swipes until all installations were completed. At that time, he discovered that 22 of the 30 sensors showed leakage of tritium. The individual involved in the activity is being bioassayed by the Army. The results of that bioassay are not yet available. The site Radiation Protection Officer was not aware that these devices were being commissioned prior to the activity. The licensee does not have a procedure for the initial installation that requires the personnel to open the packages in the hood. The procedure is written only for the removal of the sensor after use and requires the work to be conducted in a hood. The licensee is currently writing a procedure for installation. Contamination surveys conducted in the area after the leakage was discovered ranged from approximately 300,000 dpm/100 square centimeters on the buggy used to hold the muzzles, up to 1,000,000 dpm/100 square centimeters inside the hood. The area has subsequently been decontaminated to background levels. Per the RSO, these sensors are contained within a triple mylar bag during storage. At the allowable leak rate per day, tritium may build up inside the bag while stored, and when it is subsequently opened for use after extended storage, contamination may occur. The licensee has instructed its staff not to use devices if they have been in storage. This information is current as of 10:50 a.m. (CDT) July 19, 1996. NPPD is searching for a permanent replacement. Until a permanent replacement is identified, Guy Horn, Senior Vice President of Energy Supply, will assume the duties of Vice President, Nuclear. His office will remain at the corporate office in Columbus, Nebraska. On July 21, Phil Graham, the current Senior Engineering Manager, will become Site Manager. Effective July 21, Mike Boyce, the current Support Engineering Manager, will act as the Senior Engineering Manager. The engineering positions are located on site.
