The NRC is issuing this generic letter to (1) discuss the periodic verification of the capability of safety-related motor-operated valves (MOVs) to perform their safety functions consistent with the current licensing bases of nuclear power plants, (2) request that addressees implement actions described herein, and (3) require that addressees provide to the NRC a written response to this generic letter relating to implementation of the requested actions. The package contained thallium-201; however, the inner contents did not appear to be the source of the package contamination. Initial wipes of the package read up to 4 milliRoentgen per hour (mR/hr). After removal of the contents which was found to be not contaminated, detailed surface surveys of the box revealed readings up to 3 mR/hr. The package originated from the manufacturer in Billerica, Massachusetts, was shipped to a pharmacy in Valley View, Ohio, and then shipped to the licensee in Pulaski, Virginia. Upon identifying the problem, the licensee notified the courier, the manufacturer and the Commonwealth of Virginia. The contaminant and its origin are unknown at this time. Regional Action: Regions I and III, and the Department of Transportation have been notified. Region I is following up with the manufacturer in Billerica, and Region III will follow up with pharmacy in Valley View, if necessary. Contact: C. M. MR Number: 4-96-0100 Comanche Peak 2 Date: 09/18/96 Glen Rose,Texas PHONE CALL FROM RESIDENT INSPECTOR Dockets: 50-446 PWR/W-4-LP Subject: COMANCHE PEAK UNIT 2 TRIP Discussion: Comanche Peak Unit 2 tripped from 100 percent power on September 18, 1996, at 12:45 p.m. (CDT). A lightning strike during inclement weather caused two channels of reactor average temperature to spike high. These signals input to the overtemperature/N-16 setpoint calculation which caused the trip setpoint to spike low. Two of four overtemperature/N-16 trip setpoints were reached which then caused a reactor and turbine trip. All safety-related equipment functioned as required. The plant was stabilized in Mode 3. The licensee expects to restart the unit today. Unit 1 experienced a spike on one channel of reactor average temperature indication during a severe storm on August 9, 1996, which caused a reactor trip because a second channel was already tripped for testing. In addition, a dual unit trip was experienced in 1995 as a result of lightning during severe weather. Regional Action: The resident inspectors are monitoring the licensee's restart efforts. MR Number: 4-96-0102 Palo Verde 1 2 3 Date: 09/18/96 Wintersburg,Arizona Phone Call from Resident Inspectors Dockets: 50-528,50-529,50-530 PWR/CE80,PWR/CE80,PWR/CE80 Subject: CORPORATE REALIGNMENT AND MANAGEMENT CHANGES Discussion: On September 18, 1996, the Board of Directors for Arizona Public Service Company announced that the corporation is being realigned into five business units with Palo Verde Nuclear Generating Station aligned to the generation unit. The corporate realignment results in management changes that are effective immediately. Bill Stewart, previously Executive Vice President, Nuclear, assumed the position of Executive Vice President, Generation, and is responsible for electric generation and environmental, material, and facility services. Jim Levine, previously Vice President, Nuclear Production, assumed the position vacated by Mr. Stewart with a new title of Senior Vice President, Chief Nuclear Officer. Greg Overbeck moved from the position of Vice President, Nuclear Support, to the position of Vice President, Nuclear Production. Jack Bailey moved from the position of Vice President, Engineering, to the position of Vice President, Nuclear Support. Bill Ide, previously the Director of Operations, assumed the position of Vice President, Engineering.
