Jon R. Rupert will fill the new position of Engineering and Support Services Manager. Reporting to Rupert will be the Engineering and Materials Manager, the Site Support Manager, the Business and Work Performance Manager, Manager of Projects, and the Program Manager. MR Number: 3-96-0030 Zion 1 Date: 03/18/96 Zion,Illinois RI VIA PC Dockets: 50-295 PWR/W-4-LP Subject: TRUBINE TRIP AND REACTOR TRIP DUE TO HIGH LEVEL ON THE "C" STEAM GENERATOR Discussion: On March 16, 1996, at 10:56 p.m. (CST), Unit 1 tripped shortly after being placed on-line at 10:15 p.m. due to erratic operation of the 1C feedwater regulating valve, 1LCV-FW520, causing the level to increase to its high steam generator water level setpoint of 70 percent which caused a turbine trip and subsequently a reactor trip. The licensee plans to achieve criticality on March 18, 1996, at approximately 10:15 a.m., and synchronize to the grid at midnight. The resident inspectors will monitor startup activities. MR Number: 4-96-0026 Palo Verde 2 Date: 03/18/96 Wintersburg,Arizona Licensee Dockets: 50-529 PWR/CE80 Subject: REFUELING OUTAGE Discussion: Effective at 12:05 a.m. (MST) on March 16, 1996, Palo Verde Unit 2 entered its refueling outage. Major activities planned for the outage include inspection of the steam generator tubes and modification of the steam generator shrouds.
