MR Number: 1-96-0102 Calvert Cliffs 2 Date: 11/18/96 Lusby,Maryland SRI PC Dockets: 50-318 PWR/CE Subject: AUTOMATIC TRIP DUE TO LOW STEAM GENERATOR LEVEL Discussion: On November 17, 1996, at 10:59 am, Unit 2 tripped automatically on low steam generator water level. All control element assemblies (CEAs) fully inserted and operators stabilized the plant in Mode 3 (hot standby), using the auxiliary feedwater pumps to restore steam generator level. The initial investigation revealed that a retaining screw in the 22 feedwater regulating valve (FRV) position feedback linkage had sheared. A false full open position was seen by the FRV controller, which attempted to return the FRV to its proper position, resulting in full closure of the FRV. Additionally, the 22 main feedwater pump tripped on high discharge pressure, then indicated it had reset. The operators could not trip the pump from the control room due to several failed relays. The pump, however, had not restarted and was subsequently isolated locally. The 21 main feedwater pump ramped back as designed, but was tripped by the operators to facilitate steam generator level recovery. All other plant responses to the transient were as expected and within normal parameters. The licensee intends to remain in Mode 3 while performing several corrective maintenance items, repairing the 22 FRV position feedback linkage and evaluating the 21 FRV for similar problems. Contact: Henry Lathrop (410)586-2626 Fred Bower (410)586-2626 Lawrence Doerflein (610)337-5378 _ REGION II MORNING REPORT PAGE 2 NOVEMBER 18, 1996 Licensee/Facility: Notification: MR Number: 2-96-0115 Ameristeel Date: 11/18/96 Charlotte,North Carolina Subject: Contaminated Steel Discussion: On November 18, 1996, the North Carolina Division of Radiation Protection reported to the Operations Center (EN 31346) the discovery of krypton-85 contamination in some compressed metal duckwork at the AmeriSteel scrap metal facility in Charlotte, NC. The material was received in late 1995 or early 1996 and was originally thought to be naturally occurring (NORM) material. North Carolina previously reported recovering 2 krypton-85 sealed sources from the AmeriSteel facility. One source was received at the AmeriSteel facility in September of 1995 and the other in April of 1996. The sources were subsequently traced to a South Carolina licensee (Grace Finishing Co.) in Lancaster, SC. These sources were transferred back to Grace Finishing Company on October 30, 1996. The shipments were coordinated with the South Carolina Department of Health and Environmental Control. The State is following up with the owners of the previously identified gauges to determine if the incidents are related. Contact: R. L. MR Number: 3-96-0119 Duane Arnold 1 Date: 11/14/96 Palo,Iowa Dockets: 50-331 BWR/GE-4 Subject: PLANT RESTART FOLLOWING REFUELING OUTAGE Discussion: On November 14, at approximately 1:20 p.m. (CDT), the licensee commenced a reactor startup following the cycle 14 refueling outage. The reactor was made critical at 5:08 p.m. (CDT) on the same day. The main generator was synchronized to the electrical grid on November 16 at 8:33 p.m. The unit is currently at approximately 35 percent power with startup testing activities ongoing. MR Number: 4-96-0117 Comanche Peak 1 Date: 11/18/96 Glen Rose,Texas Resident Inspectors Dockets: 50-445 PWR/W-4-LP Subject: UNIT 1 REFUELING OUTAGE COMPLETION - UNIT STARTUP Discussion: On November 16, 1996, at approximately 9:22 p.m. (CST) the licensee syncronized Unit 1 to the grid, ending their refueling/maintenance outage. Major activities that took place during the outage included replacement of a reactor coolant pump, chemical cleaning and eddy current testing of all steam generators, main turbine overhaul, and numerous design modifications, including digital feedwater and electrical inverter modifications. The outage was completed in 42 days, approximately 81 hours behind the licensee's original schedule. Regional Action: For information only.
