Major activities to be performed during the outage include replacement of a leaking recirculation pump mechanical seal, replacement of a leaking safety relief valve pilot valve, and replacement of a voltage regulator on the plant's main electrical generator. In addition, during the downpower the plant will be scrammed at low power (approximately 10 percent) in order to obtain scram time data for approximately 50 percent of their control rods. They will use this data to determine the severity of the degradation of the Viton scram solenoid pilot valve exhaust diaphrams and will replace these diaphrams during the outage if necessary. Date: 04/17/96 Indian Point 1 2 RI PC Buchanan,New York Dockets: 50-003,50-247 PWR/W-4-LP,PWR/W-4-LP Subject: CONTAMINATED LIQUID SPILL IN UNIT 1 Discussion: On 4/10/96, Con Edison operators drained approximately 2900 gallons of contaminated water from a Unit 1 waste distillate tank (WDT), which is used to support Unit 2 operations. The activity of the water was approximately 10E-4 uCi/cc and required additional processing prior to discharge to the Hudson river. The WDT pumps are normally used to transfer water from the WDTs to the waste collection tanks, but automatically trip at 30 percent tank level, which was the current WDT level. Operators used the floor drain system to gravity drain the WDT water to the Unit 1 Chemical Services Building (CSB) sump where it would be pumped to the waste collection tanks. However, the Unit 1 floor drains direct the water to sump tank T-26. Sump tank T-26 is designed to pump collected liquid to the waste collection tanks. However, the tank level transmitters have been out of service since 1990, and as a result the tank's pumps have not been used to transfer tank contents since that time. Instead, a one inch drain line was used to drain the tank to the Unit 1 CSB sump. Due to the large volume of water introduced into the floor drains and sump tank T-26, the one inch drain line could not handle the volume and the sump tank T-26 filled to capacity and the water backed up in the floor drain lines and overflowed into the 33 foot elevation. The backup from the floor drain lines ran into the hallway and down the stairwells and elevator shaft to the 24 and 14 foot elevations. Con Edison cleaned and decontaminated the affected areas. There was no offsite release as a result of this spill. Con Edison is investigating the personnel errors and equipment problems that led to this event. Preliminary investigation indicates deficiencies related to procedure use and adherence to the plant modification processes. Regional Action: The resident inspectors are monitoring activities. Mr. J. A. Stall resigned as Station Manager to take a position with another utility. His other assignment becomes effective May 13, 1996. Mr. W. R. Matthews, Assistant Station Manager, Operations and Maintenance, was selected to replace Mr. Stall as Station Manager effective May 1, 1996. No replacement has been selected for Mr. Matthews. Contact: George Belisle (404)331-4196 _ REGION II MORNING REPORT PAGE 5 APRIL 17, 1996 Licensee/Facility: Notification: United States Coast Guard MR Number: 2-96-0030 Marine Safety Office Date: 04/16/96 Savannah,Georgia Discussion: On April 15, 1996, State of Georgia representatives were notified by the USCG Marine Safety Office (MSO), Savannah, regarding a radioactive shipment received at the Port. During normal rounds of the Port, a MSO Officer noted that boxes of radioactive material were improperly placarded, with 15 of the 24 boxes exhibiting bulged panels or buckled doors. Each box contained 36 fifty-five gallon drums of yellowcake consigned to Allied Signal Corp., Metropolis, Ill. A safety zone has been established maintaining the material secure and isolated. State personnel who responded to the site determined that no contamination had occurred. There is no information at this time that any drum has been breached. The State of Georgia is doing additional investigation. The shipment consignors were: Navoi Mining & Metallurgy of Combinat Russia and Kazakhstan State Atomic Power and Engineering Corp. Edlow International is the listed shipment broker. At approximately noon on April 17, 1996, an additional shipment of 22 boxes is scheduled to arrive at the Ocean Terminal. Representatives of the State of Georgia are onsite and will witness the unloading. Regional Action: Region II staff has been in contact with State of Georgia response members and is monitoring the State's findings and resolution of any violations. MR Number: 3-96-0045 Perry 1 Date: 04/16/96 Perry,Ohio SRI VIA PC Dockets: 50-440 BWR/GE-6 Subject: PLANT SHUTDOWN TO REPAIR FEEDWATER LEAK Discussion: ON APRIL 14, 1996, AN OPERATOR NOTED THAT A FEEDWATER LEAK HAD BECOME SIGNIFICANTLY WORSE FROM ONE OF TWO FLOW INSTRUMENT SENSING LINES ON A FLOW BALANCING LINE FOR THE MOTOR DRIVEN REACTOR FEED PUMP (MDRFP). THE INSTRUMENT LINES HAD NO ISOLATION CAPABILITY, AND AN INITIAL ATTEMPT TO ISOLATE THE MDRFP WAS UNSUCCESSFUL DUE TO THE SUCTION VALVE LEAKING BY. THE LICENSEE CHOSE TO REDUCE REACTOR POWER TO ABOUT 14 PERCENT, TAKE THE MAIN GENERATOR OFF THE GRID, LOWER FEEDWATER PRESSURE AND TRY REPOSITIONING THE MDRFP SUCTION VALVE. IN ADDITION, A TEMPORARY MODIFICATION WAS APPROVED WHERE THE AFFECTED INSTRUMENT LINES WERE DISCONNECTED AND REPLACED WITH PIPE PLUGS. THE REPAIR EFFORT WAS SUCCESSFUL. HOWEVER, THE MDRFP REMAINS OUT OF SERVICE DUE TO A PROBLEM WITH THE SUCTION VALVE MOTOR OPERATOR AND THE POTENTIAL FOR WATER IN THE LUBE OIL RESERVOIR. AT 1:47 A.M. ON 4/16/96 THE MAIN GENERATOR WAS RESYNCHRONIZED WITH THE GRID AND A POWER INCREASE COMMENCED. WHILE THE LEAK WAS ACTIVE, WATER AND VAPOR ESCAPED THE BUILDING THROUGH A CLOSED SET OF DOORS AND VENTILATION LOUVERS. INITIAL REVIEWS AND SURVEYS INDICATED THAT THERE WAS NO EVIDENCE OF AN UNMONITORED OFFSITE RELEASE FROM THIS EVENT AND ONLY MINOR LEVELS OF CONTAMINATION WERE DETECTED BOTH INSIDE AND OUTSIDE OF THE BUILDING. MR Number: 4-96-0040 Grand Gulf 1 Date: 04/14/96 Port Gibson,Mississippi Resident Inspector Dockets: 50-416 BWR/GE-6 Subject: UPDATE ON THE STRIKE BY SECURITY OFFICERS AND INFORMATION RELATED TO THE CONTRACT DISPUTE Discussion: This morning report updates information described in an April 3, 1996, morning report. On April 15, 1996, at 9 a.m. (CDT) the Claiborne County Men For A Better Community conducted a meeting at the plant entrance to show their support for striking security officers. Entergy is not directly involved in the contract dispute; however, the utility did establish a strike contingency plan and the contract security company did provide replacement workers for the strike that began April 7. An article in a local paper, dated April 14, further indicated that the President of the Claiborne County Men For A Better Community had "concerns that the replacement workers would not be sufficiently trained to handle emergency situations at Grand Gulf." In a similar but unrelated matter, the contract between Entergy and International Brotherhood of Electrical Workers Local 605 was extended from April 15 to May 15, 1996, to allow for negotiations on the new proposed contract to proceed in an orderly fashion. Regional Action: On Saturday and Sunday, April 6 and 7, 1996, a security inspector from Region IV evaluated the implementation of the licensee's strike contingency plan for the security force strike. The inspector concluded that the strike contingency plan contained sufficient provisions and the replacement officers received sufficient training to enable them to respond in an emergency situation at Grand Gulf.
