MR Number: 1-96-0068 Limerick 1 Date: 07/16/96 Philadelphia,Pennsylvania SRI PC Dockets: 50-352 BWR/GE-4 Subject: PLANT SHUTDOWN EXCEEDING 24 HOURS TO DUE STEAM LEAK Discussion: On July 15, 1996, operators shut down Limerick Unit 1 due to an unisolable steam leak associated with the condenser steam jet air ejectors. That morning, plant personnel observed a steam leak coming from the area of the steam jet air ejectors. Operators were unable to identify the exact source of the leak due to the inability to safely enter the area of the leak. Operators reduced power to approximately 50 percent, and swapped from the "B" to the "A" air ejector, in order to try to isolate the leak. After the swap, they were still unable to identify the exact source of the leak and isolate it, so plant management decided to shut down the plant to depressurize the steam supply line. The reactor was scrammed at about 6:17 p.m. that evening, without any complications. The plant reached cold shutdown conditions on July 16, 1996. The cause of the leak was determined to be from a plug that had blown out of a Y-strainer in a line common to both steam jet air ejectors. The licensee has formed a root cause team to investigate the failure of the plug. While shutdown for the steam leak repair, the licensee will replace the "D" traversing incore probe indexer, repair several minor feedwater pump leaks, replace the "H" safety relief valve, and repair an inoperable outboard main steam isolation valve room temperature element. The plant is expected to remain shutdown for three to four days. The licensee is planning a press release. Effective July 12, 1996, Lon Waldinger, ANO General Manager, Plant Operations, will assume the position as Director, Operations Support, at Echelon, the Entergy corporate office in Jackson, Mississippi. Randy Edington, ANO Unit 1 Plant Manager, will fill the position as General Manager, Plant Operations, created by Mr. Waldinger's departure.
