(CDT) ON 04/12/96 Discussion: The Clevite Corporation is on the SDMP (Site Decommissioning Management Plan) list, and is currently undergoing a site remediation. On the morning of April 10, 1996, a long time employee of the Clevite Corporation brought a box of miscellaneous items to the office from his home. After arriving at the office, another employee indicated that a bag that was amid the items in the box appeared to contain radioactive material. The bag was subsequently taken to the radiation technicians at the site involved in the site remediation. Surveys indicated that there was no external contamination on the bag. However, readings of 12 milliRoentgen per hour (mR/hr) (3.1 uC/kg/hr) on contact and 28 microRoentgen per hour (uR/hr) (7.2 nC/kg/hr) at three feet were measured. The State of Ohio was notified by Region III of the incident. The State of Ohio dispatched an individual to the employee's home to conduct radiation surveys. Surveys of a shed in the employee's back yard where the material had been stored, as well as inside his home, identified no radioactive contamination. Wipes and soil samples collected would have to be analyzed, but were also expected to indicate no contamination was present. The employee stated that during the 1970's the company, then under the name of Gould, was being sold. In conjunction with that sale, all the equipment was being auctioned off. The employee recognized that this material was radioactive so he wrapped it in tin foil and took it home to prevent it from being taken during the auction. He placed it in his shed where it was forgotten until recently. The material is currently stored onsite at Clevite by the contractor conducting the remediation. The contractor will assume responsibility for disposal of the material. Regional Action: Region III is waiting for the State of Ohio to provide the results of the wipe and soil sample analyses. MR Number: 4-96-0038 Comanche Peak 1 2 Date: 04/15/96 Glen Rose,Texas SENIOR RESIDENT INSPECTOR Dockets: 50-445,50-446 PWR/W-4-LP,PWR/W-4-LP Subject: UNIT 1 STARTUP FROM SHORT OUTAGE; UNIT 2 REACTOR COOLANT PUMP MOTOR FAILURE RESULTS IN EXTENSION OF REFUELING OUTAGE Discussion: Events: 30269 and 30272 Unit 1 restarted from a brief forced outage (to repair a weld crack in the safety injection system and to repair a heater drain system leak) on April 13. During the outage, rod drop testing in response to the requests of NRC Bulletin 96-01 was satisfactorily completed. On April 11, with Unit 2 in Mode 4 during a refueling outage, the Reactor Coolant Pump (RCP) 2-01 motor breaker tripped. The licensee's initial troubleshooting found that the RCP motor Phase A windings were shorted to ground. The cause of the failure is still under investigation by the licensee. The licensee returned Unit 2 to Mode 5 and plans to replace or rebuild the RCP 2-01 motor. This additional work will extend the second Unit 2 refueling outage by about 35 days with expected completion on May 11, 1996. Regional Action: The resident inspectors will continue monitoring licensee activities for the Unit 2 refueling outage. Contact: A. T.
