Johnson, NMSS (301) 415-7887 E-mail: tcj@nrc.gov _ REGION I MORNING REPORT PAGE 2 NOVEMBER 15, 1996 Licensee/Facility: Notification: New York Power Authority MR Number: 1-96-0101 Fitz Patrick 1 Date: 11/15/96 Lycoming,New York FACSIMILE Dockets: 50-333 BWR/GE-4 Subject: INCORRECT CONTROL ROD DRIVES REMOVED Discussion: On November 12, 1996 during the refueling outage, the licensee identified that 3 control rod drives (CRDs) had been mistakenly replaced. It was subsequently determined that during the work preparation phase, the wrong CRDs had been identified for removal due to a coordinate system identifying error. The job scope was to remove and replace 21 control rod drives which had been previously uncoupled. The error occurred because the CRDs to be changed out had been improperly identified and the second check failed to recognize the identification tagging error. To identify which CRDs were to be removed, the vendor had used a core map and used the undervessel doorway as a reference point. Apparently, the core map was oriented incorrectly. During a subsequent critique, it was noted that there was some discussion among vendor personnel concerning labels on the position indication probes (an indication that the CRDs had been improperly identified), but this matter was not brought to the attention of NYPA management. In parallel with the CRD replacement, work was being conducted on hydraulic control unit vent valves. Problems with draining the vent lines were occurring which prompted additional investigation and led to the identification of the above error. The licensee has developed a recovery plan to rebolt the CRD mechanisms that were mistakenly untorqued. The correct CRDs have been identified and are currently being removed. The plant is currently in cold shutdown, defueled, with a refueling outage in progress. Regional Action: Resident inspectors are continuing their review and following outage related activities. Contact: Gordon Hunegs (914)739-9360 Curtis Cowgill (610)337-5233 _ REGION III MORNING REPORT PAGE 3 NOVEMBER 15, 1996 Licensee/Facility: Notification: MR Number: 3-96-0118 Paducah Gaseous Diffusion Plant Date: 11/15/96 Paducah,Kentucky VIA RESIDENT REPORT Dockets: 07000001 Subject: BREACH OF FACILITY SECURITY GATE Discussion: On November 14, 1996, at 1:35 p.m. (CST), a passenger vehicle gained unauthorized access to the restricted area at the Paducah Gaseous Diffusion Plant (PGDP). The vehicle entered the plant through Post Number 48 on the eastern side of the plant. The roadway through the gate connects a main PGDP avenue (Tennessee Avenue) with a public road (McCall Road). Plant staff stated that the gate was open to allow cement trucks inside the plant to come and go. A security guard posted in a guard shack adjacent to the gate witnessed the vehicle pass through the gate at approximately 30 miles per hour. The guard called for help over the security radio network. Site security forces responded and were eventually able to stop the vehicle near the C-400 Decontamination Facility, approximately in the center of the plant. The occupant was identified as a plant employee's daughter who was on her way to pick up her father's paycheck. Site security escorted the individual and her vehicle to the unrestricted area after ascertaining that the driver had a valid license and registration, and that her access to the restricted area had been inadvertent. In addition, health physics performed a survey of the vehicle to ensure it wasn't contaminated before release. The gaseous diffusion plant is under the jurisdiction of the Department of Energy. The NRC has resident inspectors at the Paducah facility in preparation for the NRC assuming regulatory jurisdiction over the site in March 1997. Regional Action: Information Only.
