Headquarters Daily Report MAY 14, 1996 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION I MAY 14, 1996 Licensee/Facility: Notification: MR Number: 1-96-0049 Sikorsky Aircraft Date: 05/10/96 Stratford,Connecticut Dockets: 03003779 License No: 06-02269-03 Subject: SOURCES INVOLVED IN HELICOPTER CRASH Discussion: On May 9, 1996, a H53E Marine Corps Aircraft helicopter (still owned by Sikorsky Aircraft Corporation) crashed onto the Sikorsky Aircraft Flight Field located in Stratford, CT. The crash resulted in four fatalities. The helicopter contained seven In-flight Blade Inspection System indicators as part of the aircraft, and all seven sources of strontium-90 (500 microcuries each) have been recovered. On May 10, 1996, the licensee's consultant and a representative from the State of Connecticut Department of Environmental Protection performed leak tests of all seven sources. Results from both agencies confirmed that the sources are not leaking. The licensee's Radiation Safety Officer informed the NRC on May 13, 1996, that their consultant performed a survey of the area and has determined that there is no contamination to the environment from this accident.
