MR Number: 2-96-0121 Oconee 2 Date: 12/13/96 Seneca,South Carolina Dockets: 50-270 PWR/B&W-L-LP Subject: VOID IN OCONEE UNIT 2 HOT LEG Discussion: During a routine reduction in Unit 2 pressurizer nitrogen bubble pressure on December 3, 1996, a small unexpected increase in pressurizer level was noted. Initial investigation into the problem indicated that there was a void in the top of the B loop hot leg. Following procedural changes, the void (approx 2000 gallons) was eliminated by fill and vent activities on December 10 - 11. There were no indications of voids in the A loop or the head. The unit's Reactor Coolant System (RCS) had been drained to mid-loop for RTD maintenance in October. Preliminary findings indicate that a vent valve was mispositioned during the subsequent RCS refill, resulting in an inadequate fill and vent of the B loop. The void would have precluded the use of the 2B Once-Through Steam Generator (OTSG) for decay heat removal (natural circulation), but did not affect the 2A OTSG or the Low Pressure Injection/Residual Heat Removal System. Currently, Unit 2 remains in cold shutdown and the licensee's investigation is still underway. Regional Action: The residents will continue to followup on this issue.
