Although the relays passed initial receipt inspection and post-maintenance testing, they subsequently would not remain in the latched condition when actuated. The Salem staff is currently investigating the exact nature of the causes, and evaluating reportability under 10CFR21. Date: 09/09/96 San Onofre 2 PHONE CALL FROM RESIDENT INSPECTORS San Clemente,California Dockets: 50-361 PWR/CE Subject: INADVERTENT ACTUATION OF FIRE SUPPRESSION DELUGE SYSTEM Discussion: On September 9, 1996, an inadvertent actuation of the fire suppression deluge system occurred in a Unit 2 cable spreading riser room, located on the 9-foot level of the control building. No equipment was affected as a result of the inadvertent actuation, and the unit continued to operate at full power. Within approximately 15 minutes, operations stopped the deluge after determining that no fire existed. The licensee estimated that between 15,000 and 20,000 gallons of water emptied into the space and flowed to adjacent spaces by passing under fire rated, but not watertight, doors. Local floor drains performed as designed but were unable to maintain the cable riser room evacuated of water. The floor drains are not designed to handle a deluge, according to the Updated Final Safety Analysis Report. The licensee believes the deluge was caused by an electrician inadvertently agitating the deluge valve, located in a hallway outside the cable spreading area, while pulling a welding cable. The licensee is reviewing the flooding analysis for the control building. The licensee also intends to perform a detailed cable walkdown of the affected spaces. Regional Action: Resident inspectors walked down the cable riser room and the Unit 2 control boards shortly after the deluge. The residents observed standing water on the floor (about 1-2 inches) and in some cable trays that did not provide for drainage. No adverse effects on the unit were noted. Contact: D.
