MR Number: 4-96-0116 Callaway 1 Date: 11/12/96 Fulton,Missouri SRI VIA Telephone/HOO/RDO Dockets: 50-483 PWR/W-4-LP Subject: ESF ACTUATION AND REQUIRED REACTOR SHUTDOWN DURING RECOVERY FROM REFUELING OUTAGE (31316, 31317) Reportable Event Number: 31316 Discussion: At 10:41 p.m. (all times are CST) on November 11, 1996, the licensee closed the main generator output breaker ending Refueling Outage 8. The outage duration was 31 days. Major work accomplished included replacement of 88 fuel assemblies, two low pressure feedwater heaters, main steam and feedwater isolation system cabinets, essential service water to auxiliary feedwater piping, and main feedwater pump discharge check valves. Additional work included the removal of 126 mechanical snubbers from inside the containment building and the installation of mechanical seals on the turbine driven auxiliary feedwater pump. Steam generator tube eddy current testing was conducted and, following evaluation of the test results, the licensee sleeved 77 tubes and plugged 51 tubes. An additional 37 tubes were replugged. During the restart on November 11, at 11:05 p.m., an unplanned feedwater isolation signal occurred at approximately 20 percent power due to a high-high level in Steam Generator A (Event Report 31316). At 2:42 a.m., on November 12, with the reactor at approximately 1 percent power, operators manually tripped Reactor Coolant Pump B when pump vibration reached approximately 18 mils. Technical Specifications required a plant shutdown to Mode 3 which was accomplished at 4:30 a.m., on November 12 (Event Report 31317).
