In response to the event and per procedural direction, licensed operators initiated actions to reduce the rod line by manual insertion of control rods. The operators mistakenly began the sequence of rod insertions with the wrong group of control rods. The operators promptly realized the error they had made and immediately began inserting additional control rods in the original preplanned sequence. The rods initially inserted were left in their mispositioned status. Operators successfully returned the rod pattern to its proper sequence within 45 minutes and reactor engineering responded to ensure that no thermal limits were violated. An assessment by engineering was completed on January 8 and it was determined that no thermal limits were violated nor were there any additional core concerns. Licensee management promptly began an assessment of the event and realized, on January 8, from written statements from operations and engineering personnel, that the operators at the controls did not promptly communicate the error they had made to the control room supervisor or the shift supervisor. Senior licensee management immediately removed the shift crew from duty and initiated an independent assessment of the event. The independent assessment is headed by the Division Manager of Quality Assurance, and two members of the licensee's Safety Review and Assessment Board (SRAB) are included on the team. The SRAB members are senior industry professionals who are not on the licensee's staff. Regional Action: The Region dispatched a senior inspector from the Division of Reactor Safety to obtain a comprehensive understanding of the events that took place and ensure that the licensee's followup actions are appropriate.
