MR Number: 2-96-0077 Saint Lucie 1 Date: 09/10/96 Florida Dockets: 50-335 PWR/CE Subject: BROKEN SCREW FOUND INSIDE RPS TRIP CIRCUIT BREAKER CUBICLE Discussion: At approximately 2:35 a.m., on 9/9/96, St. Lucie Unit 1 operating crew was conducting the RPS logic matrix surveillance test. While conducting the surveillance test, the Nuclear Watch Engineer and the Station Nuclear Plant Operator stationed at Trip Circuit Breaker #5 (GE-AK-2A-25-1) observed a portion of a broken screw, with a nut attached, loose in the breaker cubicle. The logic matrix test was immediately suspended. Subsequent investigation by the licensee determined that the broken screw came from a portion of the linkage inside the breaker cubicle. The breaker was replaced and the RPS Trip Logic was restored to its normal configuration. The licensee is performing metallurgical analyses to determine root cause of the cadmium coated set screw failure. The licensee is discussing with the vendor the potential extent of condition (i.e. the number and location of similar set screws). The root cause of the screw breaking is under investigation. MR Number: 3-96-0095 Big Rock Point 1 Date: 09/10/96 Charlevoix,Michigan SIR PC Dockets: 50-155 BWR/GE-1 Subject: SHUTDOWN DUE TO LEAK Discussion: On September 10, 1996, at 10:45 (EDT), the Big Rock Point reactor was shut down to repair a packing leak on the isolation valve for the reactor depressurization system (RDS) "D" train. The leak was discovered by an operator during normal rounds. The leak did not exceed operating limits; however, the difficulty of making on-line repairs convinced management to shut down and make immediate repairs. The forced outage is expected to last approximately 6 days. Major work includes: 1) Number one feed pump packing replacement; 2) repack all RDS isolation valves; 3) inspect and repair backup core spray check valve; and 4) replace a relay on the plant generator output breaker.
