MR Number: 1-96-0010 Miller Engineering And Testing, Inc Date: 02/08/96 Manchester,New Hampshire Dockets: 03022208 License No: 28-23457-01 Subject: TROXLER PORTABLE NUCLEAR DENSITY GAUGE RECOVERED Discussion: On February 6, 1996, at 5:15 p.m., the licensee notified the NRC's Headquarters Operations Center that a Troxler Model 3411-B portable nuclear density gauge (serial number 6838) containing approximately 8 millicuries of cesium-137 (Cs-137) and 40 millicuries of Americium-241 (Am-241) was stolen from the back of a pickup truck in Dorchester, Massachusetts (PN1-9606). According to the licensee, the gauge user was returning to the Northborough, Massachusetts, office after completing work at a job site in Rockland, Massachusetts. The source rod was locked in its shielded position and the gauge was locked in its transport case. The transport case was chained and locked to the bed of the pickup truck. When the gauge user returned to his truck, he found the cap of the pickup truck was unlocked. In addition, the gauge, transport case, chains and locks were missing. The licensee notified the Boston City Police Department and the police informed the media. The licensee also notified Campbell Pacific and Troxler, two gauge manufacturers, and requested that they place this gauge on the manufacturer's stolen gauge list. On February 8, 1996, at 12:40 p.m., the licensee notified Region I that the gauge had been recovered. Apparently, the people who stole the gauge saw the television announcement and turned themselves and the gauge in to the police. The Boston Fire Department and the licensee surveyed the gauge and found no contamination. The licensee will also leak test the gauge. The licensee is obligated to provide a written report of the incident to the NRC within 30 days. Costello (610)337-5257 Kathleen Dolce (613)337-5251 _ REGION II MORNING REPORT PAGE 2 FEBRUARY 9, 1996 Licensee/Facility: Notification: Department Of The Navy MR Number: 2-96-0012 Kadena Air Base Date: 02/09/96 Okinawa, Japan Dockets: 03029462 License No: 45-23645-01NA Subject: UNAUTHORIZED USE OF MATERIAL (DEPLETED URANIUM) Discussion: On February 8, 1996, the licensee informed Region II that an aircraft assigned to Marine Air Group 12, Kadena Air Base, Okinawa, Japan, had expended a total of 1,520 rounds of 25mm PGU-20/U ammunition onto a US Air Force gunnery range approximately 50 miles off Okinawa. PGU-20/U ammunition contains 148 grams of depleted uranium (DU) per round. The Navy Radioactive Material Permit which covers this material does not authorize firing of PGU-20/U ammunition in peacetime. The range involved is a small, uninhabited island which is a permanent gunnery range in an exclusion zone. Plans are being formulated to remediate the range. The licensee learned of the incident after a "Conventional Ordnance Discrepancy Report" was forwarded to Headquarters, US Marine Corp. The report was generated after a Marine Harrier aircraft was found to have PGU-20/U ammunition on board following a training mission to the range. The licensee has launched an investigation into the incident. Preliminary information indicates that the ammunition may not have reflected the Material Condition Code which would have restricted it from peacetime use. Pending the results of the investigation, the licensee's immediate actions included: (1) the Marine Corps issued a message to all Marine units holding PGU-20/U ammunition directing Marine Corps units not to use the ammunition for peacetime or training operations, and (2) the Naval Ordnance Center issued a Naval message reclassifying the ammunition to "Material Condition Code B," restricting it from peacetime/training use. MR Number: 2-96-0013 Mc Guire 1 Date: 02/09/96 Cornelius,North Carolina Dockets: 50-369 PWR/W-4-LP Subject: UPDATE - MCGUIRE UNIT 1 REACTOR TRIP Reportable Event Number: 29931 Discussion: On February 3, 1996, operators manually tripped McGuire Unit 1 when the 1A reactor coolant pump upper motor bearing reached the licensee's administrative limit of 195 degrees Fahrenheit shortly after concluding a routine train A safety injection slave relay surveillance test. Failure to start a second component cooling water (CCW) pump or limit CCW flow to the A train residual heat exchanger at the start of the surveillance test had resulted in CCW pressure perturbations. Following the manual reactor trip, the CCW flow control valve associated with the 1A reactor coolant pump motor was found closed, isolating cooling water to the upper motor bearing oil cooler. The valve controller was checked and found to operate satisfactorily. Discerning that the earlier CCW system perturbations could have resulted from foreign material in the valve controller's impulse line, the licensee flushed the impulse lines for each of the associated reactor coolant pump CCW flow controllers. The inner diameter of the impulse lines is 0.25 inches, which is further restricted by the site practice to throttle down on the valve controller impulse line inlet manifold valves to reduce controller hunting. Long-term licensee actions involve evaluating a better configuration for the impulse line manifold valves. In addition, the Site Safety Review Group has formed a team to further investigate and identify the root cause of the event. The unit restarted on February 4, and has not experienced any further problems with reactor coolant pump motor bearing temperatures. Regional Action: Resident Inspectors responded to the trip and will followup on the licensee's long-term actions to identify the root cause of the loss of cooling water on the bearing. Date: 02/09/96 Mayaguez,Puerto Rico Dockets: 03031873 License No: 52-25114-01 Subject: POSSIBLE MISADMINISTRATION USING A STRONTIUM 90 EYE APPLICATOR Discussion: On February 8, 1996, Region II was informed of the results of the recalibration of the licensee's strontium 90 eye applicator used for treatment of eye diseases. The recalibration by the manufacturer showed a dose rate of 53 cGy/sec rather than the 24 cGy/sec the licensee had previously used. Region II had identified, during a routine inspection on October 18, 1995, that the licensee was unable to provide adequate documentation for the dose rate being used. Region II issued a CAL on October 19, 1995, confirming that the licensee ceased using the source. The licensee then had the source recalibrated by the manufacturer. The licensee had normally administered doses to the eye in the range of 1000 to 2500 cGy. Based on the dose rate discrepancy, the licensee is in the process of reviewing all administrations using the source (approximately 100) to determine if misadministrations have occurred. The licensee will notify patients and the NRC if there were any misadministrations. Region II plans to issue a CAL confirming this action on February 9, 1996. Region II is also contacting the consultant who provided the inaccurate dose rate to determine if other sources could have been measured erroneously. Region II is working with NMSS to obtain a medical consultant to review the case. Regional Action: Region II will issue a CAL and is continuing to monitor the licensee's reviews of patient administrations. Contact: J. The reason for Mr. Fisher's departure is not known at this time and no replacement has yet been named. All of the areas previously reporting to Mr. Fisher will report directly to Mr. Lee Liu, Chairman of the Board and Chief Executive Officer, for the time being.
