Representatives from the EPA and an NRC radiation specialist visited the scrap yard on October 7 and 8, and performed radiation and radioactivity surveys, verifying safe storage of the sources and interviewing scrap yard workers. There were two sources involved, one approximately a 12 inch long, small diameter rectangular rod and a similar six inch rod. A survey of the unshielded rods by a scrap yard worker on October 3 had shown approximately 200 millirads per hour at one to two feet. It appears now that the sources are from industrial gauges, rather than brachytherapy sources. One scrap yard worker may have handled the sources for approximately six to ten minutes. EPA continues in the lead follow up, including determining the origin and disposal of these sources. Either the EPA or the scrap yard will hire a waste broker to repackage and remove the sources. The EPA and scrap yard will attempt to obtain any identifying information and will take photographs of the sources when they are packaged for shipment. The State has been kept informed of the event and follow up. The NRC will continue to coordinate with the EPA to obtain information that might identify the origin of the sources. MR Number: 3-96-0108 Zion 1 2 Date: 10/09/96 Zion,Illinois SRI Dockets: 50-295,50-304 PWR/W-4-LP,PWR/W-4-LP Subject: ZION STOP WORK ACTION Discussion: On October 8, 1996, the licensee announced a site stop work action which included both Unit 1, which is operating, and Unit 2, which is in a refueling outage. This action was in response to their inability to properly control work, as evidenced by the continuation of errors and plant events. The duration of the work stoppage is not known at this time. Necessary work which was allowed on Unit 1 included critical emergent work needed for safe operation, operator rounds, and surveillances. In-progress work which continued on Unit 2 included S/G eddy current testing, turbine work, and system auxiliary transformer work. Regional Action: The residents are monitoring the stop work actions. MR Number: 4-96-0105 Waterford 3 Date: 10/09/96 Killona,Louisiana Resident Inspector Dockets: 50-382 PWR/CE Subject: INCORRECT BOLTING MATERIAL APPLICATION FOR MOTOR OPERATED AND AIR OPERATED VALVES Discussion: On October 1, 1996, in response to questions asked by an NRC inspector, the licensee identified that 3 safety-related motor operated valves (MOVs) (2 safety injection valves and one boric acid makeup valve) had either missing or incorrect mechanical fasteners installed on critical components of the valves. The licensee immediately declared the associated train of the safety injection system and the normal boric acid makeup flow path inoperable. To bound the MOV problem, the licensee reviewed the external fasteners on 78 MOVs (56 of which are safety-related) and identified numerous additional valves with fastener discrepancies. In all cases however, the licensee determined that the MOVs remained operable. The licensee is in the process of replacing the questionable fasteners. At Waterford 3, electrical maintenance technicians perform the maintenance and testing on the MOVs. Instrumentation and control technicians and mechanical technicians do the majority of the maintenance on AOVs. Therefore, to identify the scope of maintenance craft involvement in the bolting errors, the licensee decided to also review mechanical fasteners associated with air operated valves (AOVs). The licensee inspected 50 of the AOVs and found fastener discrepancies on 37 of the valves inspected. None of the discrepancies were significant or caused the valves to be declared inoperable by the licensee. The licensee is in the process of inspecting all AOVs. Waterford 3 personnel are still attempting to identify the origin of the incorrect fasteners and are completing further reviews to determine the necessity to inspect other components. Since Entergy Operations, Incorporated, shares personnel resources, the other Region IV Entergy sites were contacted about the potential for the use of incorrect fasteners at their plant. Each Entergy site reviewed a sample of MOVs and did not identify any significant fastener issues. Regional Action: Region IV will review this issue in detail during a future inspection. Contact: L.
