MR Number: 3-96-0018 Braidwood 1 2 Date: 02/08/96 Braceville,Illinois SRI PC Dockets: 50-456,50-457 PWR/W-4-LP,PWR/W-4-LP Subject: POTENTIAL INTENTIONAL CONTAMINTION OF PERSONAL CLOTHING Discussion: On January 31, 1996, a female station laborer was working in the auxiliary building transporting clean laundry into the building. The worker removed her outer work shirt and left it lying on a railing outside of the auxiliary building but on the Radiologically Protective Area (RPA) boundary. Licensee personnel stated that the laborer forgot her shirt and went home. The shirt was found later in the day by a Radiation Protection (RP) technician. The technician surveyed the shirt and found a large grease stained area reading 30 kdpm on the inside. The station assumed that the laborer had contaminated herself and removed her shirt to leave the RPA. The laborer was locked out of the RPA by RP two days later. When she reported to RP she was shown the shirt but did not recall the stain being there when she removed the shirt. The licensee has started an investigation using the station root cause team. The licensee performed an isotopic analysis and found activation products, which are normal plant contaminants. Regional Action: The resident inspectors are following the licensee investigation.
