Major activities scheduled for this 70 day outage include a complete core offload, replacement of all main steam condenser valves, and replacement of the steam generators. MR Number: 3-96-0105 Kewaunee 1 Date: 10/07/96 Kewaunee,Wisconsin VIA RESIDENT INSPECTOR E-MAIL Dockets: 50-305 PWR/W-2-LP Subject: KEWAUNEE STEAM GENERATOR TUBING DEGRADATION Discussion: As of 10/7/96, steam generator (SG) eddy current testing (ET) of inservice Westinghouse Hybrid expansion Joint (HEJ) sleeved tubes identified that about 50 percent of the tubes had parent tube roll transition indications that could require tube plugging. ET of the "A" and "B" SGs is about 75 percent complete. Concurrently, the licensee removed the plugs from 250 plugged HEJ sleeved tubes in the two SGs and performed an examination using an alternate plugging criteria granted by NRR for Westinghouse HEJ sleeves (Delta D). The licensee determined that only 20 percent of the tubes are recoverable using the Delta D criteria. The licensee had anticipated a recovery rate of 60 percent. Based on the high number of sleeved inservice tubes with indications to date and the low number of plugged tubes that may be recoverable, the licensee has projected that between 40 and 50 percent of all SG tubes may require plugging (the SGs are currently 21 percent plugged). The safety analysis allows an effective tube plugging of 25 percent, but a recent analyses for 30 percent plugging has been completed. Evaluations are also underway to determine if the plant can safely operate at higher effective plugging limits (i.e., up to 35 percent). On September 6, 1996, the licensee had submitted a Technical Specification amendment request to allow repair of faulty tubes by laser welding the HEJ at the upper hardroll center. This would establish a new pressure boundary for the sleeve joint and establish structural integrity at the weld. Current structural integrity is established at the upper hardroll lower transition. The analysis for this repair has not yet been reviewed by NRR. The licensee requested a meeting with NRR on Thursday, October 10, to discuss their options (Westinghouse will make a technical presentation). The licensee believes its only option to restart the plant (based on current ET results), is laser welding of the HEJ tubes. Regional Action: A regional specialist is on site to review eddy current (ET) data and assist the resident inspectors in following the licensee's activities in this area. MR Number: 3-96-0106 Byron 2 Date: 10/07/96 Byron,Illinois E-MAIL Dockets: 50-455 PWR/W-4-LP Subject: UNIT 2 REACTOR STARTUP FOLLOWING SG TUBE REPAIR AND REFUELING OUTAGE Discussion: On October 4, 1996, at approximately 12:30 a.m. (CDT), Unit 2 was diluted to critical following the cycle 6 refueling outage. Following the performance of low power physics testing, the unit was placed on the grid on October 5, 1996, at 1:56 p.m. (CDT). Unit 2 shut down August 8, 1996, due to a 120 gpd steam generator tube leak. After approximately one week, the licensee transitioned to the refueling outage, originally scheduled to start September 7, 1996. During the outage, the licensee completed steam generator inspection and repair and a reactor coolant system RTD bypass loop elimination modification. The unit is continuing to increase in power. Date: 10/04/96 Cleveland Heights,Ohio TELECON W/REP. B&B WRECKING CO License No: NON-LICENSEE Subject: RADIATION ALARM ON INCOMING SCRAP METAL SHIPMENT Discussion: Region III was notified that a shipment of waste caused a radiation monitoring system to alarm at a Ohio waste landfill. B&B Wrecking Co. (B&B) removed scrap metal from an old department store. The metal was sent to a landfill, and the radiation alarm tripped. The metal scrap was returned to the demolition site. B&B staff identified sensors on HVAC ductwork that was in the load. The scrap was located in a secured area near the demolished building off Mayfair Road on the east side of the Severance Shopping Center in Cleveland Heights, OH. The scrap was secured by guards and a fence around the area. Regional Action: Region III referred this matter to EPA for followup in accordance with the draft Federal Radiological Response Plan. EPA requested assistance from the Ohio Department of Health (ODH). Additionally, Region III notified the ODH. ODH performed confirmatory surveys of the scrap metal on 10/5/96, and the maximum result at contact was 2.2 mR/hr. Readings at 3 feet were indistinguishable from background. ODH identified that the radiation was emitted from 3 smoke detectors believed to contain Am-241. The smoke detectors were placed in a 55 gallon drum near the demolished building in a secured area. Since more HVAC ducts were scheduled to be removed, ODH requested B&B to place any additional smoke detectors in the drum, and notify ODH when all HVAC work is completed.
