Each plant will have two unit managers (one for each unit) reporting to a single general manager. The general manager will report to the site vice president. Each unit will have its own operations, maintenance, and work control managers and corresponding organizations reporting to the unit manager. The engineering organization is not included in the unitization and the engineering manager will still report to the site vice president. Specific Zion personnel changes, which have not yet been formally announced, include: General Manager - Steve Lacey (formerly Beaver Valley Plant Manager) Unit 1 Manager - Rob Starkey (formerly at Entergy) Unit 2 Manager - Terry Paterson (formerly Ft. The reorganization eliminates 11 management positions and 15 supervisory positions. The licensee also announced that staff level reductions could be announced by the end of the first quarter of calendar year 1997. MR Number: 4-96-0127 Palo Verde 1 2 3 Date: 12/05/96 Wintersburg,Arizona Resident Inspectors Dockets: 50-528,50-529,50-530 PWR/CE80,PWR/CE80,PWR/CE80 Subject: INADEQUATE REACTOR TRIP BREAKER SURVEILLANCE TESTING Discussion: On December 4, 1996, Palo Verde engineering determined that individual testing of the contacts which actuate the reactor trip breaker undervoltage and shunt trip coils had not been performed. The licensee's determination was made during reviews performed in response to Generic Letter 96-01, "Testing of Safety Related Logic Circuits." Technical Specification 4.3.1.1, Table 4.3-1, Item III A, requires channel functional testing of the reactor trip breakers to include independent verification of the undervoltage and shunt trips on an 18-month frequency and following maintenance or adjustments of the breakers. The licensee determined this requirement should have included independent verification of the contacts which actuate the undervoltage and shunt trips. At 1:15 p.m. on December 4, the licensee entered Technical Specification 3.0.3 for the reactor trip breakers in all three units and invoked the 24-hour period for surveillance testing provided in Technical Specification 4.0.3. The licensee completed the required testing by 2 a.m. on December 5 and declared all reactor trip breakers operable. The licensee indicated that they had contacted the licensees of four other Combustion Engineering facilities and informally concluded that these four facilities may also not be independently testing their reactor trip circuit breaker actuation circuits.
