Effective March 1, 1996, Mr. Tricur (Richie) Venkataraman assumed the role of Acting Quality Assurance Department Manager replacing Mr. Frank Famulari who resigned from BECo to accept the Quality Assurance Manager position at Quad Cities. Effective March 4, 1996, Mr. Steven Verrochi assumed the role of Acting Outage Manager replacing Mr. Mark Potkin who accepted a Nuclear Project Manager position at the PNPS. Effective February 25, 1996, Mr. Mark Santiago assumed the role of the Operations Training Department Manager replacing Mr. Alan Shiever who resigned from BECo to accept a position as the Training Department Manager at the Cooper nuclear power plant. MR Number: 3-96-0029 Zion 1 2 Date: 03/05/96 Zion,Illinois SRI VIA PC Dockets: 50-295,50-304 PWR/W-4-LP,PWR/W-4-LP Subject: LOOSE PART UPDATE/STEAM GENERATOR SLEEVE PLUGGING Discussion: Loose Part Update: On February 18, 1996, Unit 1 completed its shutdown and began to search for a loose piece of foreign material suspected to be in the primary side of the 1B hot leg, including the 1B steam generator (S/G). The material was suspected because of loose parts detection system noise indications. The primary side of the 1B S/G was visually inspected, the loop piping boroscoped, and the 1B S/G drain line was radiographed. No foreign material was found, nor was there any physical evidence of impingement from foreign material. The licensee then installed additional loose parts monitoring sensors on the S/G, closed up the reactor coolant system, repressurized, and ran various combinations of reactor coolant pumps. The loose parts monitoring system detected noise during acceleration and coastdown of various pumps, but a loose part or foreign material could still not be located. The licensee is currently working with Westinghouse on an analysis to support restart of the unit, based on the assumption that the loose part, if it exists, remains in the 1B hot leg. The licensee estimates the material to be in one piece and weigh between one half and two pounds. Steam Generator Sleeve Plugging: On March 1, based on the results of pulled tube sleeve data from Prairie Island and a just-completed review of similar data at Zion Unit 1 at the vendor (Combustion Engineering), the licensee identified 43 sleeves with anomalous indications recorded during the last refueling outage. The affected S/Gs are the 1A, 1B and 1C with 6, 14, and 23 degraded sleeves, respectively. The affected tubes in the 1B steam generator were plugged on March 4. The licensee intends to plug all affected tubes on Unit 1 prior to restart. Return to service on Unit 1 is currently scheduled for March 15. Similar data on 25 tubes at Unit 2 is also under review; Unit 2 is operating at full power. (708)829-9629 _ REGION IV MORNING REPORT PAGE 3 MARCH 5, 1996 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-96-0024 Washington Nuclear 2 Date: 03/04/96 Richland,Washington Resident Inspector Call Dockets: 50-397 BWR/GE-5 Subject: SHUTDOWN DUE TO EXCESS GENERATION CAPACITY Discussion: On Friday afternoon March 1, 1996, at the request of Bonneville Power Administration (BPA), WNP-2 began shutting down due to excess electrical generation capacity in the Pacific Northwest. Mode 4 was reached on Sunday, March 3, at approximately 4 p.m. (PST). Abundant water supplies and above normal snowpack caused this period of economic dispatch. The agreement with BPA is that WNP-2 will not be requested to restart until completion of the scheduled refueling outage (55 days) which begins on April 13, 1996. The licensee will be performing some maintenance activities during this period of economic dispatch and is reviewing the impact on refueling outage plans.
